Job Title:Third Sector Hospital Liaison Worker- LOOPS Hospital Discharge Support Project

Grade:£26,335 pro rata (fixed point)

Hours:18 hours per weekover 4 days

Responsible to:Older People’s Service Manager

Location:Royal Infirmary of Edinburgh

Holidays:30 days, plus 12 days public holidays(pro rata)

LOOPS HOSPITAL DISCHARGE PROJECT CONTEXT/PURPOSE:

LOOPs (Local Opportunities for Older People) is an Edinburgh wide network coordinated by EVOC in partnership with a range of Third Sector organisations across 4 localities, to provide links to local opportunities for older people and support their access to community services. Health in Mind is one of the partners for the Hospital Discharge Support Project (HDSP) along with EVOC (Edinburgh Voluntary Organisations Council),the Eric Liddell Centre and Libertus.

The overarching aim of the HDSP is to facilitate timely hospital discharge, help people increase their resilience and todevelop the self-management tools and techniques necessary to prevent avoidable hospital readmissions. This is achieved through supporting people to identify and access the support and services available to them in their community by Third Sector Organisations.

  1. Key Tasks

1.1.Based within the Royal Infirmary of Edinburghyou will work in close cooperation with local and national Third Sector providers across the city, NHS Lothian and City of Edinburgh Council Health and Social Care staff, to support the discharge process.

1.2.Manage your caseloadin a way that ensures your work is person-centred, prioritising the needs and outcomes of people accessing the service.

1.3.Work as part of a team to develop and deliver shared internal administrative and clinical systems and processes to support expedited referrals, as well as enhanced and accelerated transitions back to the community.

1.4.Ensure that all functions of the project’s work positively contribute to future developments of Health and Social Care Locality Hubs by participating, sharing information, and seeking feedback.

1.5.Carry out and complete appropriate assessments of needs, action planning/ person centred plans, aiding the dischargeprocess for clients.

1.6.Work closely with the newly developing volunteer-led ‘Assisted Discharge’ services provided by the Royal Voluntary Service (RVS).

1.7.Participate in and contribute to LOOPs Network Meetings where appropriate

1.8.Work to build and maintain constructive working relationships with members of your immediate team and the wider teams within Health and Social Care

1.9.Network and liaise with relevant services and agencies to ensure the provision of a comprehensive and co-ordinated supportfor our project and clients.

1.10.Continuously engage with services to promote data sharing and governance, andthe Memorandum of Understanding agreement.

1.11.Ensure that record keeping is timely, accurate and consistent, so as to facilitate production of the case studies and progress reportsthat are necessary for effective monitoring, evaluation, and reporting purposes.

Person Specification

Knowledge: Essential

  • Understanding of Health in Mind and its values
  • An understanding of the challenges facing vulnerable older people including at the time of a health crisis and delayed discharge from hospital
  • An understanding of the barriers preventing vulnerable older people accessing services
  • Knowledge of local services for older people in Edinburgh

Skills: Essential

  • Excellent presentation skills
  • Excellent communication and interpersonal skills
  • Numeracy skills- to produce a summary of results.
  • Excellent IT skills- inputting into a database, intermediate word, and basic excel
  • Good influencing and negotiation skills
  • Ability to manage change
  • Working across sectors including statutory and private sector service delivery
  • Self-motivated, able to take initiative and to work independently within the hospital environment
