A. Purpose
What is the reason for doing this project? Briefly sketch the background leading to your proposal, critically evaluate any existing knowledge, and identify the specific gaps that your project is intended to fill. Clearly state the importance and health relevance of your proposal and relate these specific aims to the broad, long-term objectives. If the aims of your project are achieved, please state how clinical practice in the area of Adolescent Transitional Care will be advanced. Please describe the impact your project will have on patient care and services that comprise the field of transitional care.
B. Specific Aims
Give an exact and concise statement of what you intend to accomplish with this project and how your project will improve patient care at LPCH for adolescent patients? How does your project meet the mission of the Innovations in Adolescent Transitional Health Care Program?
C. Background and Significance
Briefly sketch the background leading to the present application, evaluate existing knowledge, and identify the gaps that the project is intended to fill. Describe the problem that you are addressing. Describe any work you have previously done that is related to this proposal. How is this important to adolescent and young adult health care? What is the current situation within and outside of LPCH? What impact might your project have on the problem? Please describe the design or clinical framework, procedures, and analyses to be used to accomplish the specific aims of your project. If applicable, include how the data will be collected, statistically analyzed, and interpreted. Please describe any novel concepts, approaches, tool, or technologies for the proposed study.
D. Design & Statistical Methods
Please describe the design of your project: Define your study population; describe inclusion and exclusion criteria. How will you recruit and enroll subjects into your project? Clearly describe the procedures or interventions used once a subject is enrolled. Clearly define the measures or interpretive approaches you plan on using. What are your primary outcome variables? Is your sample size large enough to achieve your goals? What other variables (e.g. descriptive, process) will you evaluate?
E. Data Analysis
How will you collect, report, and evaluate the measures? Or if this is a qualitative study, describe what type of analysis will be used regarding the phenomena you are studying.
F. Potential Pitfalls
Please describe the potential pitfalls and limitations for your project such as patient recruitment, limitations to methodology, etc. How will you handle them? Discuss alternative approaches to achieve the project aims. Please note any anticipated HIPAA and IRB compliance issues.
G. Future Steps and Scalability
Lastly, please define your project follow-up work: How you will measure success? Is there long-term sustainability? How will you scale the project to serve others? How this project will lead to new opportunities for research funding
