Explanation of Forms

REV. MICHAEL J. BADER MEMORIAL SCHOLARSHIP FUND

VIRGINIA STATE COUNCIL * KNIGHTS OF COLUMBUS

Form BF1:

  • This form is for the student and parents to fill out. Do as completes as possible and be sure to use additional paper, if necessary, to list activities in Church, Community, and School. Competition looks at both the financial need and the student’s involvement.

Form BF2:

  • This form needs to be completed by the Financial Secretary of the Father’s Council. The Financial Secretary should affix the Council’s seal on the document when signing it to attest to the Father’s standing in the council. Be sure that the Financial Secretary submits it early enough to arrive in the Chairman’s hands by March 14th if you do not mail it back yourself.

Form BF3:

  • The High School Principal or their representative must complete this form. This is the form that will attest to the student’s grades. Please ensure that this form is returned to the Chairman by March 14th or the student will not be eligible for competition.

General Notes:

  • Please file early to assure that the forms arrive in time. The deadline is March 14th. Any forms received after that date will not be considered.
  • All three (3) forms must be received by March 14th in order to be considered for the scholarship.
  • If you are leaving forms BF2 and BF3 for the Financial Secretary and Principal to return, it is wise to get the forms to them early and check back with them to ensure they have mailed them in plenty of time to reach the Chairman.
  • Competition is based on Grade Point Average, Financial need, and the student’s involvement in activities in their Church, Community, and School. Be detailed, the more information available, the easier to determine this information.
