Purpose

This form is directed to instructors who have students that perform minimal risk research as part of the course activities. The instructor can request to be a delegate reviewer of the research by completing this form. The form will also assist the instructor in determining if the course activity requires a full REB review submission form. To qualify for this type of delegation, the student research must never be published or used in any context beyond the instructor and the students enrolled in the course. For example, the research may not be presented at conferences, or be included in a journal article.

Please save this form to your hard drive and fill in the form using Word. Applicants can either submit a hard or scanned soft copy of their completed application package. Submit the electronic copy of the completed form together with supporting materials as attachments (questionnaire instruments, interview questions, consent forms, recruitment materials, debriefing forms, safety approvals, etc.) to . A hard copy of the completed application package can be dropped off or mailed to SAIT Research Ethics Board, SAIT Polytechnic, MA323, 3rd Floor, Heritage Hall, 1301-16 Avenue NW, Calgary, Alberta, T2M 0L4.

G. COMPLIANCE

●If students’ research is not directly reviewed by the SAIT Research Ethics Board, it can not be used for any purpose other than the associated course assignments. In other words, it cannot be published.

●I understand that I am responsible for reviewing the students’ research so that it conforms to the ethical principles outlined in the TCPS2. If I am not clear on the principles contained in the TCPS2, I can take a free tutorial at this website:

●If a student’s research plan is unethical, I will not allow the research to proceed.

●If there is a question about the ethical nature of a research project, the project may be submitted to the REB for a formal review.

●If any of the information contained on this form changes, I will submit a new form.
