STORAGE AND ADMINISTRATION OF INJECTABLE

AND NONINJECTABLE MEDICATIONS

POLICY:

Injectable psychotropic medications may be administered only by certain licensed staff. Injectable medications are the only medications administered by EFCMHC staff.

PROCEDURES:

Medication storage:

All Injectable medications and noninjectable medications given by the nurse/RN, shall be kept in a separate, designated, locked container.

Access to the designated locked container is limited only to authorized clinic staff members, as determined by the physician.

The storage container will contain written procedures for administration of medication including:

a. A standardized list of abbreviations and symbols.

b. Procedures for control and accountability of medication.

1) All medications shall be properly labeled.

The label includes the name and address of the pharmacy, date of filling, names of consumer and prescriber, directions for administration, prescription number, the trade or generic name, quantity, and strength.

All professional sample Injectable medication shall be labeled to show name of drug and strength.

2) Packaged prescription medication that has a change of order shall display a sticker that states that there has been a change in the order.

3) A licensed nurse will be responsible for routine inspection and control of all drugs in storage, including inspections, to ensure the quality of drugs.

4) A medication destruction form will be maintained.

c. Antidote charts and telephone numbers of state poison center.

d. A list of personnel currently authorized to administer injections that shall be updated as changes occur.

e. A list of personnel authorized by the physician to have access to the storage area.

f. A list of physicians authorized to prescribe medications.

g. A current inventory of drugs shall be maintained by a licensed nurse on site.

h. Storage conditions will be adequate in terms of light, moisture, sanitation, temperature, and ventilation, according to state and federal guidelines or laws.

i. Adequate materials (syringes, alcohol, bandages, gloves, Sharps Container) will be stored in the designated storage area to safely administer the injections.

Medication Administration

Medication may be administered only with an authorized physician's written or verbal order.

a. Telephone or verbal orders may be recorded in the consumer's records by licensed nurses only, and shall be used only in emergency situations. The responsible physicians shall sign the telephone orders within 48 hours.

b. Administration of the Injectable medication will be limited to the responsible physician or to a designated licensed nurse.

c. Injections will be prepared and administered in designated areas of the clinic which provide privacy.

d. The clinician who administers the injection shall follow "Universal Precautions" to prevent transmission of blood-body fluid, HIV, and other blood-borne pathogens, including:

1) wearing of gloves throughout the procedure.

2) hand-washing before and after procedure.

3) placement of syringe, without breaking needle, in puncture-resistant container for disposal.

4) Health care worker shall not administer injection if he/she has exudative skin lesions.

Safety Precautions

a. After receiving their initial injection of a particular medication, consumers will be asked to remain for observation for an appropriate period of time (usually 20 minutes).

b. Trained personnel (physician or nurse) will be immediately available who can recognize adverse reactions and initiate treatment. All direct care staff will be attentive to the possibility of medical emergencies related to use of medication. Staff will be trained at initial Orientation and annually concerning medical emergencies. Staff will be familiar with Policy & Procedure III-26 Medical Emergencies.

c. An EFC physician will supervise the preparation of an emergency kit which is readily available, but accessible only to staff. This kit will contain such equipment and medications as deemed necessary by the physician to treat anaphylactic reactions or other common adverse reactions.

d. The contents of the emergency kit will be listed and the kit will be inspected on a monthly basis by a designated staff member to insure completeness of contents. Outdated, deteriorated drugs and supplies will be replaced during the inspection. The monthly inspection will be documented.

e. The emergency kit will be stored in the designated storage area with the Injectable psychotropic medications and will itself be sealed with a separate lock.
