Certified Addictions Counselor

Part-Time 0.4 FTE

(6 Month Contract with possible renewal based onfunding)

In the North York Family Health Team (NYFHT), physicians work with a team of healthcare professionals such as registered nurses, nurse practitioners, chiropodists, dietitians, social workers, pharmacists and physician assistantsto offer the very best possible healthcare to our community of patients. Family Health Teams are a key piece of the Government of Ontario’s plan to build a healthcare system that promotes good health, reduce wait times and improve access to care.

JOB DESCRIPTION:

The NYFHT Addiction Counselor will work with patients suffering from substance abuse problems and other addictions. In this role, you will support the development of recovery programs and coping strategies to avoid relapses. In addition to individual sessions you may conduct sessions with family members and those identified as support to the patient. In some cases, you may refer a patient to the psychiatrist or other medicalprofessionals.

KEY RESPONSIBILITIES:

The successful candidate will be responsible for providing services to patients and families with addictions (including opioid addiction):

  • Outreach and engagement with clients
  • Pre-and post-treatment support
  • Assessment and referral
  • Individual counseling
  • Development of case plans based on client-centred recovery goals
  • Networking with community agencies
  • Crisis/conflict intervention
  • Client documentation
  • Community follow-up
  • Team consultation and decision-making
  • Travel within NYFHT boundaries of care provision

QUALIFICATIONS:

  • CACCF/ICADC Certification preferred
  • Degree from a recognized institution in one of the human services professions preferred (qualification to enter a Provincially Regulated College a strong asset)
  • Minimum 3 years’ experience working in the addiction/mental health sector
  • Education/training in harm reduction, opioid replacement therapies and overdose prevention
  • Knowledge and experience in assessments, individual and group counseling and case management
  • Understanding of trauma and PTSD
  • Knowledge of culturally competent practices
  • Excellent written and spoken English—other languages an asset
  • Maintain client records / files according to agency standards
  • The ability to work individually and as part of a team
  • The ability to work collaboratively with community partners
  • Knowledge and experience with data management systems
  • Strong work ethic, initiative and commitment to delivering positive outcomes
  • Successful police check as per NYFHT Policies and Procedures

Salary: $27.50 per hour
