IMPORTANT NOTE
1)If this performa in all respect as required under the rules is not submitted in triplicate to the Organising Secretary in time, the concerned team shall under no circumstances be allowed to participate in the corresponding Tournament before permitting the team to participate it is a must to ensure that :
i)Eligibility details and the identity cards not have any unauthorized over-writing.
ii) The performa on which the eligibility details have been submitted must bear the stamp of the Department of Physical Education, University of Calicut.
iii) The identity cards must bear the photograph of the player and the signature duly attested by the Principal, Identity cards must be produced before the commencement of each game.
2)The performa filled in and duly signed by the Principal is to reach the Organising Secretary, Sub Convenor, atleast 2 hours before the commencement of the first match in games. In athletics the performa must be submitted as soon as the team reports at the venue of the meet.
3)The organizing Secretary/Sub Convenor will scrutinize this performa and satisfy himself that the members of the team are eligible to participate in the tournament.
