This post will join the existing team of seven Consultants across Clyde. The post is for a Consultant General Physician with an interest in Gastroenterology based at Inverclyde Royal Hospital. Sub speciality interests will be supported.

Inverclyde Royal Hospital is a modern district general hospital in Greenock, serving the population of Inverclyde, Largs, Bute and the Cowal Peninsula. All major specialties are represented within the hospital. The hospital has a progressive agenda in relation to clinical standards, managed clinical networks and patient focused service re-design. There are close clinical links with the Community Health Care Partnership, the other acute hospitals within Greater Glasgow and Clyde, and the University Hospitals in Glasgow.

Greenock is situated approximately 40 minutes from Glasgow, amid superb coastal and mountain scenery, and with excellent transport links.

Inverclyde Royal Hospital has a team of nine Consultant Physicians, 3 Specialty Doctors, fourteen Middle Grade Staff and twelve FY 1 Doctors.

The post holder will be required to maintain and develop the gastroenterology service and work with colleagues to progress service delivery for patients across Clyde.

MRCP UK or equivalent qualification is essential and you must have full registration with the GMC and a licence to practice. Those trained in the UK should have evidence of higher Specialist Training leading to a CCT in Gastroenterology and General Acute Medicine or eligibility for specialist registration (CESR) or be within six months of confirmed entry at the date of interview. Non UK applicants must demonstrate equivalent training

Consultant Physician with an interest in Gastroenterology

This post of Consultant Physician with an interest in Gastroenterology will be based at the Inverclyde Royal Hospital (IRH), Greenock. The successful candidate will integrate into a team comprising seven Consultants to develop services within gastroenterology across Clyde. This will offer a real opportunity for the successful candidate to be involved in the development of a successful modern consultant led service for a population of c 380,000.

1. General Information

Inverclyde Royal Hospital is a modern district general hospital in Greenock, and serves the Inverclyde District (Greenock, Gourock and Port Glasgow), Largs, Fairlie, Skelmorlie, Wemyss Bay, Bute and the Cowal Peninsula, a total population of approximately 120,000 comprising both urban and rural populations. The hospital was opened in 1979 and currently has 342 beds.

All major specialties are represented within the hospital, with a new 90-bedded Geriatric Hospital opened in February 2001 and an 8 bed Physically Disabled Rehabilitation Unit that opened summer of 2002.

The hospital is justifiably proud of the close inter departmental links and cooperation. The hospital is at the forefront of a progressive agenda in relation to clinical standards, managed clinical networks and patient focused service re-design. There are close clinical links with the colleagues in the Community Health Care Partnership and other hospitals in Greater Glasgow and Clyde, and the University Hospitals in Glasgow.

There is a dedicated hospital wide academic programme supported by a strong departmental educational commitment. The hospital is well provided with a dedicated education center and IT support. The hospital has an excellent reputation in supporting and nurturing its junior staff and has an excellent reputation for post graduate training.

Local Amenities

The local district has excellent facilities for most sports including golf, swimming, rugby, cricket and tennis. In addition to the sports and swimming / leisure centres, there are great opportunities for those who are interested in sailing, as some of the best sailing in the world can be found in the Clyde estuaries. Largs Yachting Marina lies 12 miles to the south, offering RYS sail training courses. Ferries giving access to the Highlands and Islands of Scotland sail regularly from Gourock and Wemyss Bay, within a 5 – 10 minute drive from the hospital.

There area also major shopping centres in Silverburn and Braehead which are within reach, and one of Europe’s premier shopping cities is about 25 miles away. There is a good transport infrastructure with rail, bus and motorway links as well as close proximity to 2 international airports; Glasgow International Airport and Glasgow Prestwick International Airport.

Greenock is situated on the West Coast of Scotland about 30 minutes from Glasgow, amid superb coastal and mountain scenery. The area offers excellent residential accommodation and first class state and private schools.

2. The Medical Unit

The Medical Unit consists of an Acute Medical Receiving Unit and three general medical wards totaling 116 beds. In addition, there is an 11 bedded Coronary Care Unit, and a Haematology Day Bed Unit and a 16 station chronic Haemdialysis Unit which is part of Regional Services. The Medical Unit provides an acute service in general medicine with clinics in general medicine, diabetes, endocrinology, cardiology, asthma, respiratory medicine and haematology, in addition to gastroenterology.

Medical services are also supported by teams of specialist nurses in all specialties including chest pain, heart failure, diabetes, respiratory , oncology, haematology, stroke, alcohol liaison etc. There is good social work and community liaison through a bed management team and discharge planning nurses .

4. Consultant Staffing

In addition to the current post, there are nine consultant physicians with special interests as noted.

The Medicine for the Elderly Department has three consultants and is currently integrating with the Department of Medicine at both Consultant and middle-grade level. The three Consultant Geriatricians contributes fully to the on-call medical rota.

Other Consultants that support the Medical Unit are detailed below:

Consultant Haematologists, Dr M G Rainey who admits haematological in-patients to the General Medical beds. There is an Haematology Day Bed Unit based in F South where day patients are seen Monday to Friday.

Four Consultant Dermatologists, D C Fitzsimmons, Dr A Milligan, Dr I Hay, and Dr C May run a busy Dermatology Ambulatory Care Treatment Centre with their own dermatology Specialty Doctors.

Dr C Geddes and Dr S Rogers Consultant Nephrologists, visiting consultants from the Western General Infirmary (WIG) visit the Renal Unit twice weekly and run Renal clinics from within the Renal Unit with the support of Associate Specialists.

Dr O Jack and Dr S Cooper, Consultant Neurologists that provide weekly clinics.

Professor M McIntyre is the Clinical Director for Clyde and Dr C Jones the Lead Clinician for Inverclyde Royal Hospital.

5. Junior Medical Staff

Junior medical staff are integrated between Medicine and Medicine for the Elderly (MFTE). Currently there are 14 Middle Grade Doctors who together with junior psychiatry doctors provide ward, emergency and outpatient support. 6 FY2s based in MFTE and 12 FY1’s proving cover in the acute receiving and general medical wards. In addition, out of hours work is supported by a Hospital at Night Team.

6.Gastroenterology Services

6.1 The Gastroenterology shares 34 beds on G South with Respiratory at Inverclyde Royal Hospital. There are a further 39 beds at Royal Alexandra Hospital.

6.2 Staffing for Gastroenterology services in Clyde will be seven Consultant Physicians and with 5 Consultant Surgeons at Inverclyde Royal Hospital.

6.3 There are Nurse specialists in Colorectal disease, Alcohol Liaison and Hepatitis C are already in post. The Nurse specialists play a pivotal role in co-ordinating an integrated service delivery to Gastroenterology and Surgical based Oncology patients.

6.4 There is a newly refurbished endoscopy suite at Inverclyde Royal Hospital which includes 2 fully equipped rooms, post operative recovery area and counselling rooms. There are two fully equipped rooms at Royal Alexandra Hospital as well as one at Vale of Leven Hospital. The dedicated nursing staff are very supportive of GI service provision and development as well as being actively involved in academic development and service developments e.g. PEG feeding management and outreach support. Computerised endoscopy reports and digital image captures are in place.

6.5 Dietetic services offer close support to gastroenterology patients through ward/outpatient services. A joint celiac disease clinic has been running for three years.

6.6 There is an alcohol liaison service.

7. Acute Medical Services

7.1 Acute Receiving Ward

Acute medical admissions are admitted to ward J North (30 beds), which is staffed by one of the consultants on a rotating ‘Physician of the Week’ basis Monday morning to Friday lunchtime, then by another of the consultants for the weekend. Patients are then triaged to the appropriate specialty wards if not for early discharge.

7.2 General Medical Wards

There are three general medical wards, J South (22 beds) G North (30 beds) and G South (34 beds) which receive patients from the Acute Medical Receiving Wards. These wards are supported by the different specialty consultant staff and their teams of junior doctors.

The general medical team are well supported by both surgical, radiological and anaesthetic colleagues with 3 ITU and 4 HDU beds on site.

7.3 Coronary Care Unit

Patients with acute myocardial infarction, acute coronary syndromes, arrhythmias or congestive cardiac failure are admitted to an 11 bedded coronary care unit. The unit is staffed during the day by one of the cardiologists on a rotational basis, and supported by junior medical staff.

7.4 Junior Doctors Out-of-Hours

A Hospital at Night service commenced in August 2006, and medical staff covering the out of hours period are required to lead and participate in this team, as well as attending the hand over. This is part of a wider work assessing the impact of modernizing Medical Careers and implementing plans to support the delivery of high quality sustainable services across Greater Glasgow and Clyde.

8. Medicine for the Elderly

Medicine for the Elderly is part of the Rehabilitation and Assessment Directorate, however many of its activities are combined with Adult Medicine. In particular, the out of hours cover is provided by junior doctors from Medicine for the Elderly and Adult Medicine.

There are 91 beds that comprise of three Medicine for the Elderly Wards (ward 1, 2 and 3), a Stroke Ward (K South), and the Day Hospital. While the unit is located next to the main hospital site, the Stroke Ward is located in the main hospital campus.

9.Management Arrangements

The Emergency Care and Medical Directorate in the IRH consists of Emergency Medicine and medical specialties. The consultant appointed will be expected to participate fully in the work of the Directorate.

The Clinical Director for Medicine across the Clyde Division is Professor Martin McIntyre, based at the Royal Alexandra Hospital and the General Manager is Miss Melanie McColgan, also based at RAH.

The Lead Clinician for medicine at Inverclyde Royal Hospital is Dr Chris Jones.

10. Summary of the Post

10.1 Duties of the Post

Gastroenterology

This post replaces a retiring consultant with a specialist interest in general gastroenterology, applicants are ideally sought from a general gastroenterologist with an interest in hepatology. The post holder will provide a consultant-led, easily accessible gastroenterology service. The successful candidate will be actively involved with the other 6 Consultants to develop an integrated, efficient Gastroenterology service across both RAH and IRH.

The post holder will perform two gastroenterology clinics (combined new and return) and the job plan will include at least one clinic and endoscopy session at the Royal Alexandra Hospital.

The post holder will take responsibility for the daytime care of gastroenterology patients, out of hours cover being provided by the physician on-call. Emergency endoscopy provision is under the care of the surgical department though the post holder is encouraged to be actively involved. The department is involved in undergraduate and postgraduate teaching and the consultant appointed will be expected to devote approximately one session per week to this. In addition it is expected that the post holder would fully participate in multidisciplinary meetings with colleagues in surgery, radiology and pathology.

Endoscopy

The consultant will also undertake two combined (Upper and Lower) endoscopy lists with responsibilities to include diagnostic and therapeutic procedures. Experience in ERCP would be a beneficial, but is not essential.

General Medicine

Each consultant is on-call for emergency admissions either Monday – Thursday or Friday – Sunday periods. This is a fixed rotation and is planned for twelve months in advance. During the week of on-call the number of emergency patients between 9 am and 9 pm is seasonal and can vary between 10 to 30 per day, with an average of 25 patients admitted.

The on-call rota will be 1:12 for weekdays and 1:12 for weekends. The medical Directorate works well as a team and actively encourages co-operation between colleagues to allow flexibility in fulfilling service responsibilities.

10.2 Suggested job plan

A work programme for the week will be negotiated and agreed with the successful candidates. The core working week will be based on 9.0 PA’s of Direct Clinical Care (DCC) duties and 1 PA of SPA time.

The job plan will be tailored to the successful candidate’s interests and the following should be taken as guidance. A detailed job plan will in due course be agreed with the Clinical Director and thereafter this will be subject to annual review and adjustment if necessary.
