NYSTART Clinical Team Leader
Program Overview:
New York Systemic, Therapeutic, Assessment, Resources and Treatment (NYSTART)will increase the community capacity to provide an integrated response to people on Long Island with intellectual/developmental disabilities and behavioral health needs, as well as their families and those who provide support. This will occur through cross systems relationships, training, education, and crisis prevention and response in order to enhance opportunities for healthy, successful and richer lives. NY START - will provide community-based crisis prevention and intervention services for individuals with intellectual/developmental disabilities (I/DD) and co-occurring mental health (MH) and behavioral health needs.
NY START – will help individual obtain adequate treatment options when they need them most in the least restrictive setting possible. STARTcomponents include:
- Trained Linkage Coordinators
- In-home therapeutic supports (ages 6 – adult)
- Site Based Therapeutic Resource Centers (ages 21 +) – planned and emergency use
- Crisis support 24 hours/7 days a week
- Team response time, 2-3 hours
- Augments existing system of support - does not replace it;
- Multi-level cross system linkages (local, statewide, national);
- Clinical education teams, online training forums;
- Family support and education;
- Standardized protocols for cross systems crisis prevention and intervention designed to connect MH and I/DD providers;
- Consultation, assessment, service evaluation;
- Employs data driven, evidence-informed practices and outcome measures;
- Technical support to maintain program integrity and fidelity to the START model;
- Person-centered focus; and
- Focus on understanding problems in the context of the system of support.
Position Summary:
Under the supervision of the START Associate Director, the START Clinical Team Leader is responsible for the supervision of the START Coordinators in support of the Long Island START team. The START Clinical Team Leader will ensure that crisis support, linkages, outreach, and consultation services are effectively provided for individuals with a developmental disability and behavioral health needs.
- Provide administrative supervision to START coordinators and proved daily clinical triage support
- Maintain linkages and relationships with community partners
- Ensure the coordination of support meetings and crisis plans for individuals served through START
- Share on-call responsibilities to ensure 24/7 response to crisis situations within one hour of call telephonically and within 24 hours of call face-to-face
- Participate in recurring meetings with START leadership, clinical team, and resource center
- Development of cross-systems crisis plans, intake/assessments, intervention and outcomes and any other applicable documentations of services provided
- Maintain a small, active caseload (about 5 cases) until START coordinator certification is achieved. Following certification, team leader caseload should be fluid and temporary to provide opportunity to support START coordinators when needed
- Complete trainings necessary to become a certified START coordinator within 12 months of employment as a team leader
Position Requirements:
- License in social work or mental health counseling (*LCSW preferred)
- 3 years’ of experience providing services to people who have IDD and mental health/or challenging behavior needs
- At least 1 year of supervisory experience
