·  Work groups review XYZ gap analysis and raise relevant questions and issues

·  Senior Leaders respond to questions and issues raised by work groups

·  Senior Leaders adjust (if necessary) and endorse final gap analysis

3:05-4:05 PM GAP ANALYSIS – WORK GROUP BREAKOUT GROUPS

·  Work groups “cascade” XYZ gap analysis and identify work-group-based gaps

·  Work groups prioritize gaps

4:05-5:00 PM INTEGRATED GAP ANALYSIS

·  Work groups present gap analyses to other groups and to Senior Leaders

·  Senior Leaders discuss and endorse integrated gap analysis or defer decision to next morning pending evening review

DAY 2

9:00-9:30 AM INTRODUCTION TO ROOT CAUSE ANALYSIS

·  Review of LCOR approach to root case analysis

·  Discussion of example – preferably from XYZ

9:30-11:00 AM ROOT CAUSE ANALYSIS – BREAKOUTS

·  Work groups build on previous day’s gap analyses to identify and prioritize root causes

·  Note: breakout teams might be cross-functional (vs. based on work groups) depending on the nature of the gaps

11:00-12:00 PM INTEGRATED ROOT CAUSE ANALYSIS

·  Breakout teams present prioritized root cause analysis to other groups and to Senior Leaders

·  Senior Leaders discuss and endorse integrated root cause analyses or defer decision to immediately after lunch pending lunchtime review

12:00-1:00 PM LUNCH

1:00-2:10 PM CASE STUDY – LEADING CHANGE
