Permission Slip

Dear Parent/Guardian:

The main teaching approach employed in Career and Technology Education courses is “authentic” learning. The students will have the opportunity to work with materials and power tools to meet the course requirements. Before any student is allowed to use any tools, a review lesson describing the proper and safe use of that tool is presented. Included in the lesson is a demonstration of the proper operating procedure by the instructor. In addition, each class spends time discussing safe working practices and proper conduct to be followed in a work setting.

I ask that you sign this sheet and then have your student return the sheet to me as soon as possible. A copy of this will be available on my teacher website. Please make sure that both sides are signed.

Richland High School Career & Technology Education Laboratory Safety Rules

General Rules:

1.  No horseplay, running, practical jokes or throwing supplies will be tolerated. Loss of working privileges will result from any infraction of this rule.

2.  Safety glasses must be worn upon entering all shop areas during work periods.

3.  Loose clothing must not be worn. Button or roll up sleeves and secure long hair when working on machinery.

4.  Rings, watches and jewelry that may catch on machinery or cause a short circuit should be removed when using equipment.

5.  Open toe shoes and sandal type shoes are dangerous to wear and should not be worn during class labs.

6.  Report all accidents, no matter how slight, to the instructor immediately.

7.  Floors in working areas and around machines should be kept free of paper, rags, oil, grease, water, and other materials from which a person could trip or slip.

8.  Bench tops and storage cabinets should be kept clean and orderly so material will not fall and cause injury.

9.  All dirty rags should be placed in self-closing, airtight, metal container.

10.  Every student is responsible for lab cleanup.

Tool and Machine equipment Rules:

11.  No student is to operate any machines or new machine equipment without being first instructed on its proper use.

12.  Never, under any circumstances, operate any tools or machine equipment if the instructor is not present in the classroom.

13.  Never clean a machine while it is in motion.

14.  Never use dull or damaged equipment. Report such things to instructor immediately.

15.  Never stop a machine with your hands.

16.  Never leave machinery running when not in use.

17.  Students will not crowd around machines while someone else is operating it.

19.  Do not distract attention or talk to anyone when they are operating a machine.

Both sides of permission slip must be signed and dated.
