Please see this as a constructive exervise and provide direct and specific feedback. The feedback can be about any aspect of your training post but we ask you to focus on matters that relate to your educational experience during this placement.

Please note that this form has two parts. The first part of this document (page 1-6) is confidential and will only be viewed by BME staff and will NOT be viewed by your Supervisor. Page 7 asks for feedback on your GP Placement and this page will be viewed by your Supervisor. If you have any issues or concerns that you do not wish to include in this assessment but with to raise with BME, please contact your Education & Training Coordinator, Local Medical Educator or Director of Training at the BME office in your state.
