Job Description: SOAR Benefits Specialist

Overview: The SOAR Benefits Specialist is responsible to work with other members of the PATH team and use the SOAR Model with eligible clients. SSI/SSDI Outreach, Access and Recovery (SOAR) is an approach that helps increase access to mainstream benefits for people who are homeless or at risk of homelessness. This person will help clients remove personal and environmental obstacles to access benefits and coordinate all aspects of appropriate claims.

Education/Experience Preferred: BA/BS in Human Services; experience working with people experiencing homelessness and mental illness; excellent writing and computer skills; ability to work with a group or independently; experience in the provision of social services.; highly motivated and creative with excellent interpersonal skills; ability to provide comprehensive, intensive services to individuals experiencing homelessness; must have valid driver's license and access to reliable transportation with proof of insurance.

Responsibilities:
Complete training including web based and refresher courses run by SOAR State Leaders.
• Provide outreach and refer to direct access to treatment for both mental and substance use disorders.
• Provide case management via evidence based practices to link with and retain clients in housing and other necessary services, including but not limited to primary care services, and to coordinate these services with other services provided to the client.
• Encourage an array of integrated services and supports for individuals with substance use disorders, mental disorders, or co-occurring mental and substance use disorders.
• Maintains on-going client files, including care plans, correspondence, case notes and PATH SOAR report.
• Assist clients in identifying un-addressed needs related to benefits, education, housing, medical and other needs.
• Assist in program development and quality assurance activities as directed by supervisor.
• Attend required trainings programs and community meetings as assigned.
• Develop a strong community profile within the homeless services and community as an advocate for clients
• Attend regularly scheduled supervision, staff and or other meetings as requested.
