RELEASE FORM: I/We, the parent/guardians of the above mentioned, hereby give my/our approval to participate in all GNW baseball/LL softball activities. I/We assume all risks and hazards incidental to such participants including transportation to and from the activities; and I/we do hereby waive, release, absolve, indemnify and agree to hold harmless GNW/LLS, the volunteers, board members, participants and persons transporting my/our child to or from activities, from any claim arising out of an injury to my/our child, except to the extent and in the amount covered by accident or liability insurance. I/We authorize any licensed physician to provide treatment, order injections, hospitalize, give anesthesia or perform surgery for: (child)______. I/We understand that this authorization given prior to any need for medical care, but given to avoid unnecessary delay in emergency treatment which the physician may deem advisable in the exercise of best judgement. I agree to return, upon the last game of the season, the uniform and or equipment issued to my child in as good a condition as when I received it except for normal wear and tear. A $25.00 fee for non-returned uniforms will be accessed and paid by me to GNW or LLS if I do not return this GNW or LLS product back to GNW or LLS.

CODE OF CONDUCT FOR PARENTS AND PLAYERS
PARENTS
· Please realize there are different goals for interleague and tournament teams.
· Support your child and coach.
· Be positive with your child and his/her teammates.
· Be respectful to game officials, they are usually volunteers.
· Key on the positives not negatives.
· Most children forget losing very quickly – you should too.
· Make sure players get to the games and practices on time. If there is a problem, let the coach know in advance. Otherwise, playing time can be affected.
· NO smoking, tobacco products, alcohol or drugs during any league activities.
· ALL complaints shall be directed to the Cal Ripken/Lou Leaver League Town Representative first, not to the coaches or other workers.
PLAYERS
· Make sure you get to games and practices on time. Let the coach know if you will be late or absent, and explain why. Otherwise, playing time can be affected.
· Show respect to your coaches, teammates, opponents, and game officials.
· No smoking, tobacco products, drugs or alcohol.
· Take care of the fields and the facilities by wearing clean sneakers and using the equipment properly. Do not liter.
· Show good sportsmanship. Name calling and swearing will not be tolerated.
· Attend practices with the intent to learn by paying attention. No fooling around.

NO TOLERANCE POLICY
Great North Woods Cal Ripken Baseball/Lou Leaver Softball Leagues have established a NO TOLERANCE POLICY towards unsportsmanlike behavior. Any PLAYER, COACH OR SPECTATOR who verbally disagrees, abuses or loudly expresses oneself can be ejected, penalized or removed from the premises. This may be done without warning and will be strictly enforced. Any person violating this code of conduct will be disciplined at the discretion of the Board.
Most of the officials and workers are volunteering their time. Please respect their efforts in putting on a quality program for the children.
I have read and agree to abide by the Code of Conduct for Parents and Players, as well as Great North Woods Cal Ripken Baseball/Lou Leaver Softball’s NO TOLERANCE POLICY. By signing below I understand I am signing on behalf of all family members.
