A Problem/Solution Document

Problem:

As a new teacher/consultant for the deaf and hard of hearing, I was overwhelmed with the numerous and diverse responsibilities that came with the position. One of the most challenging, yet basic and most necessary, responsibilities was that of documenting my visits I made with my student and their staff. I had students that received direct service, students that received only technology support, and yet other students for whom I solely consulted with staff. How was I going to document when I met with the student and/or staff, what I actually did during this visit, and that my actions during this visit were directly related to their IEP goals and objectives?

Solution:

I had created several documentation forms just for this purpose. None ever seemed to fit the bill in every respect. So, after scrutinizing my original documentation forms as well as numerous colleagues’ forms, I devised the forms that I still use to this very day. Please see them below and feel free to use them in any way that will make you more effective in your job!

Impact:

These forms have made my job easier in terms of documentation and reporting. I am able to see the progress of the students on one single form (per marking period). Thus, when reports are due I am able to simply review and restate what had been accomplished in terms of IEP goals and objectives without hesitation or question. It is also great to be able to quickly see when and what kind of visitation was made. Additionally, my supervisors appreciate the succinctness yet comprehensiveness of the information.
