3.Acopyofthisformshallbeplacedinthefacultymember’spersonnelfile.

1.Afacultymembermayappeal,inwriting,totheBORVicePresidentforHumanResourceswithinten(10)calendardaysuponreceivingwrittennoticethattheoutsideworkwasnotincompliance.
2.ThefacultymembershallreceiveawrittenresponsefromtheBORVicePresidentforHumanResourceswithinten(10)calendardaysstatingthereasonsforthedecision.
3.ShouldthefacultymemberdisagreewiththedecisionoftheBORVicePresidentforHumanResources,thematterwillbesubmittedtotheOfficeofStateEthicswithinten(10)calendardaysfromthedaythefacultymemberreceivestheresponse. TheBORVicePresidentforHumanResourcesmayelecttosubmitthematterdirectlytotheOfficeofStateEthicsforitsopinion. ThiselectionbytheBORVicePresidentforHumanResourceswouldsatisfytheobligationtorespondasstatedinparagraph#2.
