Dear Provider,
The NYSADSP Board of Directors is pleased to once again send out applications to our provider members for nominations for The Leadership Award, and for the Service Provider of the Year Award.
This year we have revised our submission criteria and there is no limit on the number of applicants that an agency can submit. Each agency can submit nominees from each category;
The Service Provider of the Year Award is for those staff that provide direct support or direct services for consumers in some capacity. Please nominate someone who has demonstrated exemplary performance over the course of his/her employment, who has been dedicated to providing quality service and promoting independence.
The Leadership Award is for those who provide supervision of Direct Support Professionals. Please nominate someone who has provided strong and consistent leadership for direct support staff and exemplifies these qualities consistently. *Please see the definition page attached.*
In order to nominate staff, the agency must be a member in good standing with the Association. Each agency can nominate employees for each award.
There will be one winner chosen from each of the six NYSADSP Regions. (Six winners in all for each award) All winners will be recognized at the 2011 Symposium held in November at the Gideon Putnam in Saratoga Springs. Each winner will receive free Symposium registration and one night lodging for the Symposium.
Please be sure your agency strongly considers worthy nominees from your Day Habilitation, Day Treatment, Pre-Vocational, Supported Employment or OPTS program – it is a great way to recognize those exemplary staff - send in your nomination(s) - preferably by e-mail -to me by August 25, 2011.
