How does your company maintain records of equipment/plant/tools? What inspection criteria are used? Is anyone in the company trained or certified for inspection works, if so identify the inspection/certifying body. What tool/plant/equipmentis inspected and what is the frequency?

11.3Certification Process

Does your Company have a register of tools/plant/equipment that needs to be certified as per legislation? Are records maintained and do copies of certificates accompany the tool/plant /equipment to the worksite?

11.4User Competence

Is specific equipment/plant/tool training provided? Is this updated? Does your company have records of validity? Are ongoing assessments of operators undertaken?

12.1The Selection Process

Is there a process to assess their HSE capability before selection? E.g. review from the past records, recommendation from previous hirer, HSE report/ records, HSE performance and etc.

12.2Monitoring of Contractors

Are regular checks conducted of the worksites (E.g. from Welfare facilities, Tools and Equipment, Personnel conditions and etc.? How is this recorded?)

12.3Induction Training & Competency.

Have new employees been given a company induction. Moreover, how does the company evaluate their employee’s competency level?
