NEW YORK STATE LICENSURE EXAMINATION SUMMARY: (updated March, 2014)
You can initially only apply for licensure in one state. Information on the application process in other states is in their website, Google (state name) physical therapy licensure process or something similar.
New York State Education Department Application and fee includes license and 3 years of registration. The packet described by Dr. K includes all the forms necessary for applying for licensure in New York State. These forms are only available on line at:

NYS FORM 1 - Application for Physical Therapy License -
Sent to NYS Office of the Professions.

FEE IS $294.00 and payable to the New York State Education Department.
NYS FORM 2 - You complete page one, print your name again in Part A of page two, and submit to Dr. Krasilovsky. He will forward this form to the Registrar’s office, who will complete this form after graduation and completion of all affiliations and then send it to the NYS Education Department. If you complete your affiliation later than usual, be sure to call Dr. K upon completion to remind him to submit this form to the Registrar.

Note that the exam is given only four times per year. The best dates for Hunter graduates are July 22 & 23rd, 2014. Register sooner rather than later, although your application will be considered incomplete until they receive the Form 2 from the Graduate Center the week after graduation.
Once processed, you will receive more information about the schedule for the exam. Results will be received in about one month after taking the exam, or go to the NYSED web site listed above and see if you are listed as being licensed.
When scheduling or taking the exam, you are asked if results can be sent to your school indicating your name. We would appreciate this information so we can analyze how our alumni do on the exam.
Limited Permit: ($70 fee). You must apply for a limited permit if you want to work before you pass the licensure exam. You can work up to 90 days without having physically received the limited permit, as long as you have applied for it. The limited permit is good for six months, and if you have not taken the licensure examination within six months, you will have to show progress toward taking the exam to have the limited permit extended. See page 9 of NYS PT Licensure Application packet. You can only apply for a limited permit once you know where you will be working.
