2. 3. Travel costs (trips, hotels)2)

b) shall be refunded against relevant receipts/invoices up to the amounts defined in relevant regulations on business trips;

c) shall be paid by UAM (including per diems for foreign visitors or per diems catering).

§4

Performance of the work under this contract shall be approved by ……………………………………. A relevant note on the Contractor’s invoice is required.

§5

1. Without UAM’s prior written consent the Contractor cannot subcontract all or part of the work stipulated in §1 to any third person.

2.Even if the prohibition stipulated in §5(1) is breached, any third person will be prevented from the performance of any work under this contract and will not be paid any remuneration.

3.In the event when the provision stipulated in §5(1) is breached, UAM may terminate the contract forthwith and stop any payments to the Contractor.

§6

  1. Any non-UAM employees must complete a statement stipulated in Appendix 6 to UAM President’s Regulation No33/2012/2013 of 26 November 2012, which constitutes an appendix to this contract.
  2. If any data given in the statement, which constitutes an appendix to this contract, is changed, the Contractor shall notify the relevant employee of the Dział Płac i Stypendiów UAM [Salaries and Scholarship] and will re-submit an updated statement, clearly indicating which data has been changed. The updated statement must be submitted forthwith, however not later than within 3 working days from the change.

§7

Any amendments to this contract shall be made in writing in the form of an annex, otherwise being null and void, and will be effective when signed by both parties.

§8

Any disputes, which may arise out of or in connection with this contract, shall be settled by competent courts in the registered office of UAM. Any matters, which have not been stipulated herein shall be governed by the relevant provisions of the Polish civil code.

§9

The contract has been made in two identical copies, one for UAM and one for the Contractor.

The Contract has been concluded (underline and complete):
