SFSP - Health Department Notification Letter
Instructions:
- The sponsor is required to notify the local health department of:
- The planned summer meal service sites before site operations begin.
- New sites, closed sites, and meal service changes (dates, days, times, meals) as they occur throughout the summer.
- The sponsor is required to submit a copy of the initial health department notification letter with the Sponsor Application Packet.
