EMED Participating Teachers Assurances Form
Participating teachers should have the following qualifications:
· Teacher 4th grade
· Implements research-based instructional strategies in math
· Has a high level of educational technology expertise
Each participating teacher must agree to the following:
· Attend 75 hours total hours of professional development each year for two years (2009-10 and 2010-11) including three professional development days (substitutes provided by the grant). Attendance at the following conferences is also required and will be covered by the grant:
o Utah Rural Schools Conference – July 6-10 2009 in Cedar City
o Utah Council for Teachers of Mathematics Conference (UCTM) – October, 2009
o Utah Coalition for Educational Technology (UCET) – March, 2010
o Utah Council for Teachers of Mathematics Conference (UCTM) – October, 2010
o Utah Coalition for Educational Technology (UCET) – March, 2011
· Receive frequent classroom visits from EMED instructional coaches and the project director.
· Participate in collaborative learning communities – face-to-face and through WIMBA.
· Assume responsibility for use and care of equipment provided
· Agree to stay in present school teaching 4th grade math for 2 years
· Participate in evaluation activities including providing student CRT data.
· Complete a minimum of one individual Podcast to contribute to the project
Each participating teacher will receive the following:
· $1,700 stipend each year upon completion of 75 hours of professional development, creation of assigned podcasts and appropriate classroom implementation of technology-enhanced math instruction
· One laptop (Mac Book Pro or Windows-based)
· One Flip video camcorder
· One document camera
· One iPod Touch
· One headset microphone
· Professional Development credit (USOE or SUU) applied towards an educational technology and/or elementary math endorsement.
Each participating school administrator agrees to support participating teachers by:
· Providing release time to attend required meetings and conferences
· Allowing the teacher to remain assigned to teach 4th grade mathematics for the two years of the grant as feasibly possible
· Giving priority to EMED professional development if conflicts arise
· Providing additional resources as needed
Each participating district agrees to support participating teachers by:
· Providing an LED projector for each participating teacher’s classroom.
· Providing needed technical assistance for all hardware purchased by the grant.
· Providing access for the grant director to necessary CRT data.

Note: Equipment purchased through the grant is the property of the School District. Participating teachers have the right to use the purchased equipment during the term of the grant, and while they are employed by the grant district.
