NO ENTRY FEE IS NECESSARY TO PARTICIPATE OR WIN. THE CONTEST IS VOID WHERE PROHIBITED BY LAW, REGULATION, STATUTE, OR OTHER APPROPRIATE LEGAL AUTHORITY.

I.  Purpose

1.  Raise awareness about mental health and wellness of California’s kindergarten through twelfth grade (K–12) students;

2.  Recognize the efforts of Project Cal-Well to address the mental health needs of K–12 students;

3.  Reduce stigma associated with mental illness.

II.  How to Enter

1.  Contestants must e-mail the completed, signed Contestant Entry and Release Form and the contest logo design by April 30, 2016, to the California Department of Education (CDE) at . Faxed or mailed submissions will not be accepted. Late entries will not be considered.

III.  Contest Requirements

Each contestant may submit only one logo.

The logo design must include the name “Project Cal-Well,” must incorporate the color lime green, which represents mental health awareness, and must creatively embrace one or more of the following Project Cal-Well goals:

·  Increasing awareness of student mental health issues;

·  Providing training for school staff, parents, and community partners to identify and support students experiencing emotional distress;

·  Improving school climate;

·  Connecting students to appropriate school or community-based mental health services.

Logos should be submitted in PDF or JPEG format.

IV.  Eligibility

1.  The contest is open to students in grades seven through twelve who are California residents. Contestants under the age of eighteen must have a parent/guardian co-sign the Contestant Entry and Release Form. Submissions will be accepted through April 30, 2016.

2.  The person submitting the logo design will be considered the contestant and will be the only person eligible to compete for the prize. In order to be eligible to win the prize, the contestant must provide accurate and complete information on the Contestant Entry and Release Form. Incomplete or inaccurate entry forms will not be considered.

3.  The following individuals are not eligible to enter the contest:

·  Project Cal-Well Employees Family Members

V.  Prizes

The contest will have three winners: Gold, Silver, and Bronze. A panel will be selected and convened by the CDE to review the design submissions and determine the winners.

The prize winnings for each category are:

1.  Gold ($500);

2.  Silver ($300);

3.  Bronze ($200).

Winners will be notified in writing and the winning entries will be posted on the CDE Project Cal-Well Web page as soon as the winners are determined.

VI.  Limitations of Liability and Release

By submitting a logo design, you are indicating that you have read and agreed to the following items:

·  You will not submit any logo designs that you believe to be unlawful, harmful, threatening, abusive, harassing, defamatory, vulgar, obscene, libelous, invasive of another’s privacy, hateful, or racially, ethnically, or otherwise objectionable. You acknowledge that CDE has the right, but not the obligation, in their sole discretion to pre-screen, refuse, or discard any submissions it deems to be unlawful, harmful, threatening, abusive, harassing, defamatory, vulgar, obscene, libelous, invasive of another’s privacy, hateful, or racially, ethnically, or otherwise objectionable.

·  You will not submit any logo designs with content that you do not have a right to make available under any law or other proprietary rights of any party.

·  You authorize CDE and its Designees unlimited use of your logo design, at any time, and in any manner deemed appropriate by the CDE. You acknowledge CDE’s right to alter or edit the logo design in any manner deemed necessary by the CDE.
