Use this tool to document your team’s transition goals for the coming year. Some goals have a more rapid implementation than others, so this may be a short term, or a long term plan of action. The action steps are the primary steps that need to be taken in order to achieve the desired goal. It is desired that a Transition Team have two to three goals for a program year, however in some cases it is a big task to get even one new program, or change in place for a school site. Goal 1:

Action Step 1: Our group has always had resource fairs at the school but we have noticed that the attendance has greatly reduced. We discussed how maybe the school setting was not the most favorable because some of the parents might not view this setting as the most comfortable. We felt too that sometimes it is an overabundance of material. Another concern was how our parents are not educated on what is to happen when these kids graduate. The parents are used to the school or DRS making sure that everything is taken care of. We have decided to have quarterly meetings at key schools (so that the distance will not be so far). These will be informative meetings with presenters along with information for that subject that is being presented. Our first one is at Sapulpa at Central Technology Center on December 13, 2016. We will have food and prize drawings also. Krsheka and I will present the first night. The next one will be on Social Security and Guardianship. We meet with Bristow tomorrow to set up a meeting night for them, Kellyville, Depew, Stroud, and Davenport. The reason that Davenport and Depew are in there is because they are part of the Co-Op and I have some students from there. Oh and we hoping to assist with the transportation issues….. Sapulpa is going to have busses run that evening to pick up families that do not have transportation.
