The 2016 CSLP Annual meeting will be held at the Radisson Hotel Salt Lake City Downtown, 215 W South Temple, Salt Lake City, Utah, (801) 531-7500.

Attached is a brief tentative agenda along with the registration form, lodging instructions, shuttle instructions, proxy forms, and suggested sightseeing activities from the area. Please note the meeting start times and ending times on the draft agenda are firm however items within the agenda may be moved around prior to the beginning of the meeting. Just a reminder, this meeting is provided to you by your peers and you are expected to remain through the entire meeting. Leaving early jeopardizes the impact and the knowledge gained by this annual meeting. CONFERENCE LODGING:

All lodging reservations must be made directlywith theRadisson Hotel Salt Lake City Downtownno later thanMarch 1, 2016. All guests will need to guarantee lodging on a credit card for incidental items. Rooms will be$139.00/night(plus tax) for single/double. (Any rooms requiring triple or quad occupancy will be an additional $10per person plus tax per room, per night) All sleeping rooms include complimentary wireless Internet access. If you are wishing to arrive early or stay later, it is recommended that you phone in your reservation. The room rates will be based on availability. The additional days will be at your own expense and must be reserved with RadissonHotel Salt Lake City Downtown to guarantee availability.

The Radisson Hotel Salt Lake City Downtown is a non-smoking hotel.
The CSLP Board of Directors has approved the following CSLP expenditures for this conference:

  • One room per state or islandmember for up tothree nights (Tuesday-Thursday).

State room use will be determined by your state representative and may be used by anyone fromyour state.

  • One room per member of the Board of Directorsfor up to four nights (Monday – Thursday)
  • Up to two (2) representatives per state/island will receive meals and refreshments on Tuesday through Thursday. (For those who cannot fly out on Thursday evening, reservations will be made at The Red Iguana Mexican restaurant. CSLP will cover this expense.) A list of area restaurant options will also be available at the annual meeting.)
  • Additional meeting attendees (over 2) per state or islandmemberwill be charged $400 to cover meals, refreshments, and meeting costs.
  • A stipend of up to $700 will be reimbursedto one person from each state/island to help defray travel expenses not reimbursed by their state/island.Receipts are required for all reimbursements.Travelvouchersneed to be submitted to the CSLP office no later than Thursday, April 29, 2016. Reimbursement for your expenses will be issued in May 2016. Reimbursable items include: Airfare, airport parking, luggage charges for one bag to/from Salt Lake City, Utah, transportation to/from Salt Lake City International Airportto the Radisson Hotel Salt Lake City Downtown(See options on next page), mileage to/from the departure airport, and meals on travel days. All meetings will be held in the Radisson Hotel Salt Lake City Downtown.

Salt Lake City International Airport (SLC)
TheRadisson Hotel Salt Lake City Downtown is approximately 10 miles from the Salt Lake city International Airport (SLC). The airport operates out of two terminals. Delta operates out of Terminal Two; Alaska, American, Frontier, JetBlue, Southwest, and United operate out of Terminal One. To see more information visit airport website at:

Shuttle Service to and from the airport and hotelThere are multiple ways to obtain transportation to and from the airport and hotel. CSLP will pay up to$25 for one-way transportation. Remember you must have receipt of this expense for reimbursement. The airport website has various shuttle options or you may take the light rail – TRAX for approximately $2.50 one way. HOTEL PARKING:

The Radisson Hotel Salt Lake City Downtown has an attached facility. The cost is $12 per night for self-parking or $15 per night for valet parking. This charge will be for annual meeting nights only and may be a part of the reimbursable stipend of up to $700 total.
