RISK ASSESSMENT FORM AND GUIDELINES:

Please complete the Risk Assessment Formbelow using BOTH:

-i) the guidelines shown below, and

-ii) any specific guidelines that you are aware are particularly applicable to your own business and activity. Note that various guidelines are available on the Website to assist you.

You should consider what risk there is

-for those setting up your stand,

-for those who are working on your stand, and

-for the general public during the Event.

Even if in your opinion there is, in respect of a particular hazard or situation no risk please nevertheless fill in that section stating WHY you consider there is no such risk and return the form to FEL.

Add new cells if you identify more Hazards than the standard number of cells in the table in this Form allows.

Please fill in the columns dealing with Probability and Severity to help FEL (and you) to evaluate the risks posed by each of the Hazards.

Probability / Severity
Veryunlikely / 1 / Veryminorinjuries / 1
Unlikely / 2 / Minorinjuries / 2
Possible / 3 / Injuries / 3
Likely / 4 / Majorinjuries / 4

GUIDELINES

HAZARD / WHO MIGHT BE HARMED / WHAT EXISTING CONTROLS DO YOU HAVE? / WHAT MORE MAY BE REQUIRED TO CONTROL THE RISK?
Look out for hazards that you could reasonably expect to result in significant harm under the conditions of your working environment and stand.
Here are some typical examples, but please note that FEL is asking you for your own assessment of your own business.
EXAMPLES OVERLEAF / There is no need to list individuals by name – but you must think about groups of people doing similar work, activities, or who may be affected by the Hazard that you have identified.
EXAMPLES OVERLEAF / For those Hazards that you have already identified and which you already control in some way, please list, for each Hazard, the measures that you already apply (and will apply at this Event).
What makes the hazard less risky?
What makes the controls effective?
EXAMPLES OVERLEAF / For the hazards listed, do the precautions already taken control the risk satisfactorily, or is there some further measure that you need to take?
For example, do your controls:
-Avoid
-Control
-Protect all
-Improve the effectiveness of the existing controls
