Application for Preliminary Examination

When doctoral courses and the language requirement (if applicable) have been completed, the student requests to take the preliminary examination. This examination is both written and oral. The request to take the examination must be approved by the graduate coordinator and the preliminary examination committee, and then filed in the GraduateCollege at least four weeks prior to the start of the examination.

It is at this time that the Graduate College appoints a graduate faculty representative to participate in both the examination and dissertation. However, depending upon program guidelines, the composition of the examination committee may be similar to or different from the dissertation committee.

Preliminary Examination Report

At the conclusion of the preliminary examination, the committee determines if the student has passed or failed and reports this decision to the GraduateCollege.

PLEASE NOTE: The form you have downloaded is an MS Word template and, for your convenience, can be completed on-line. Once you have filled in the necessary fields, print the form and secure the required faculty signatures and forward to the GraduateCollege.
