1. Please include a check made payable to "DTA" for $150 for all positions except Youth Members and Members-at-Large and a check for $300 for Vice-President/President-Elect position.
2. Nomination fee is not required for the Youth Members and Members-at-Large positions. Preference shall be given to graduating high school seniors for the Youth Members.
3. Nomination fee will not be refunded to if there are elections for any position.
4. Nomination must be submitted by 11:59 P.M. on August30th, 2012 using one of the following two methods. Please note that any nomination received after this time (even if the postmark date is on or before this date) will not be accepted.
Method 1: Complete the nomination form with all the requirements and email a scanned copy to . The check payable to DTA should be delivered or mailed to the General Secretary on or before 08/30/2012.

6. You must have paid your membership dues for 2012 by June 30, 2012andalso be a member during 2011or be a life member/life sponsor by June 30, 2012 in order to be eligible to be nominated for any DTA position. You must be nominated by a member and seconded by two other members as specified in the DTA bylaws.
