Midhurst Mile & Bike Rodeo BBQ

Our Midhurst Mile & Bike Rodeo is a volunteer run event to help raise funds for the Rob Plunkett Track and the Baseball Diamond at ForestHillPublic School. Come out and celebrate our school spirit with classmates, family and friends! This year we will be offering gourmet hot dogs, veggie dogs, fries, BeaverTails, soft drinks, water and freezies!
Parents are asked to please pre-pay for the selections below (by chequespayable to ForestHillPublic Schoolor cash online). Please fill out the pre-order form, and, return it to the school by Thursday, May 19th using one form per family. No sales of hot dogs and fries will be available the night of the event. Don't miss out, pre-order your food! Please pick up your pre-ordered food promptly!

Midhurst Mile Fun Run/Walk and Bike Rodeo
Thursday May 26th, 2016 @ 5:00PM
Since 2007, Forest Hill has been raising funds for the maintenance of our school track and student athletics through this spirited celebration!! This year funds will go towards maintenance of the Rob Plunkett Memorial Track and the school baseball diamond. The one mile fun walk/run is a loop through our community, starting and finishing at ForestHillPublic School. The school grounds will also feature a bike rodeo, including not-to-be-missed activities like an obstacle course for young riders, a skate park, a bouncy castle, a spinning challenge and big bike fun zone with ramps for the more experienced riders. Sponsors will be providing complimentary bike checks and servicing, and awesome raffle prizes are up for grabs. The first 100 registrants will receive a loot bag!!!
Pre-order dinner from Michael & Marion’s with a BeaverTail for dessert! No sales of hot dogs and fries, will be available the night of;BeaverTails and beverages will be available in limited quantities. Please pick up your pre-ordered food promptly.
The Forest Hill Staff and Parent Council will be honoring Leigh Ann Brown at the beginning of the Midhurst Mile. Leigh Ann was a cherished staff member and friend of the community for 20 years and passed away suddenly last June. Please join us in remembering her when we name our garden in her honour.
5:00 – Registration opens (pick up pre-ordered food vouchers and wristbands); bike rodeo and other activities begin.
5:30 – Leigh Ann Brown Memorial Garden Dedication
5:45 - Warm up with Miss Connie and O’Canada
6:00 – Race starts (road closed)
7:30 – Activities wrap up!
What you need to know -

Rain or shine!
Waivers are required for participation in all activities (please complete the attached liability waiver and return it to the school by Thursday, May 19th. Also available on the Forest Hill P.S. website). A wristband will be included in your registration package for each participant.
Return food order forms to the school by May 19th
Check in at the registration desk to pick up your pre-ordered food vouchers & wrist bands to participate
While there is NO FEE to participate, donations are GLADLY accepted and any donations in excess of $20.00 will be issued a tax receipt!
The class with the most participants at the Midhurst Mile will receive an ICE CREAM PARTY for the entire class!
Please bring your own water bottles as we will have a water cooler station on site!

FOREST HILL 2016WAIVER, RELEASE AND INDEMNIFICATION

In consideration for permission from Forest Hill Public School (FHPS) and Partners to participate in Forest Hill Midhurst Mile/Bike Rodeo on May 26, 2016 (the “Activity”), and in full recognition and appreciation of the dangers and risks inherent in the Activity, I hereby waive, release, and forever discharge FHPS and Partners(and all of its affiliates, elected officials, directors, officers, employees, contractors, agents, volunteers and those for whom FHPS and Partners is in law responsible) (collectively, the “Releasees”) from and against any and all liability, claims, demands, actions, or causes of action for costs, expenses or damages to property or personal injury (including death) (collectively, “Liability”), which may result from my or my child’s participation in the Activity due to any cause whatsoever, including without limitation, negligence, or breach of any statutory or other duty of care on the part of FHPS. I further hereby agree to hold harmless and indemnify the Releasees from and against any and all Liability which may result from, or is in any way connected with, my or my child’s participation in the Activity.

ACKNOWLEDGEMENT OF RISKS

I understand and acknowledge that participation in the Activity is voluntary. I understand and acknowledge that the Activity may be hazardous, that participation is solely at my own risk, and that I assume full responsibility for any injuries (including death) or damages resulting from my or my child’s participation in the Activity. I affirm that I am, and/or my child is, in good health and at an appropriate level of physical fitness and physical capability to participate in the Activity. I affirm that I am at least 18 years of age, or that I am the parent and/or legal guardian of the child listed below and I hereby consent to their participation and waive liability on their behalf.

CONSENT TO USE IMAGE

I hereby consent and authorize FHPS and Partners to take my photograph/image and/or the photograph/image of the child listed below, for whom I am the parent and/or legal guardian (collectively, “Image”). I acknowledge and agree that FHPS and Partners may publish, display or use the Image for any FHPS/Partners purpose and by any means whatsoever including, but not limited to, electronic or digital means. I acknowledge that FHPS and Partners may not be able to control the distribution or use of the Image by other than FHPS/Partners representatives. I agree that this consent is given in perpetuity and for no consideration, credit, acknowledgement or financial recompense, now and in the future. I hereby waive, release, and forever discharge the Releasees from and against any and all Liability whatsoever which may result from the taking, use, display, publication or distribution of the Image.

BY SIGNING THIS DOCUMENT, THE PARTICIPANT / GUARDIAN ACKNOWLEDGES HAVING READ THIS DOCUMENT AND UNDERSTANDS THAT THE PARTICIPANT / GUARDIAN IS WAIVING CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE.
