MPDP / MPSCP / MPRRP

Duly certified true copy of SEC registration, articles of incorporation and by-laws, or DTI registration certificate for single proprietorship ( for MPDP;MPSCP & MPRRP);

Copy of valid Business/Mayor’s Permit (for MPDP, MPSCP & MPRRP);

Copy of dealership agreement with Mobile Phone Distributor/Supplier duly accredited by NTC (for MPDP only);

List of stocks of spare parts and accessories sufficient enough to cover the warranty of mobile phone units for at least six (6) months, or in accordance with the dealership agreement with the NTC accredited Supplier/Distributor (for MPD)

Proof of paid up capitalization (minimum of P100,000.00 for MPDP and P50,000.00 for MPRRP) ;

Lease of stall agreement for the conduct of business activity (for MPRRP only);

Resume and employment certificate of at least one (1) qualified electronics technician who has completed a formal training course in the repair, servicing and maintenance of mobile phones and presently employed by the firm on a full-time basis (for MPSCP only);

List of service or test equipment and measuring instruments (for MPSCP only)

FEES : MPDP / MPRRP :Filing Fee – Php 500.00 MPRRP : Registration Fee - P1,500.00/yr

Permit Fee – MPDP : P2,500.00/yr; MPDP Branch: P1,500.00/yr MPDP/MPRRP :Inspection Fee – Php 1,500.00/Year

FEES : MPSCP :

Filing Fee – Php 180.00 Permit Fee – Php 1,200.00/year Inspection Fee – Php 720.00/Year
