Our CommunityLearning DisabilitiesTeamsandNeuroRehabilitationteamsprovidespecialistnursing andtherapistsupportthathelps topromoteequalaccess tomainstreamhealthservicesfor adultswithphysical andlearning disabilities. Theservicealsoprovidesdirectnursing andtherapyassistancetopeople using theservicewhoarevisitedintheir own homesor incare homes.Alimitedserviceisalsooffered tochildren.Examples oftheassistancegiventopeople andtotheirfamilies includeepilepsymanagement,managementofmedication andsupportwhenpeople exhibitbehavioursthat challengeothers.

For peoplewithalearning disabilityweoperateanopenreferralsystemtoGP-registeredpatients,accepting referralsfrom healthand socialcareprofessionals,carers andserviceusersthemselves(subject toaserviceeligibilityassessment ifrequired). Ourteams are madeup of the following professionals:
