NOTIFICATION PROCEDURES FOR ACCIDENT/INCIDENTS AND CODE 10 EVENTS

REMEMBER: ALL Incidents whether received via phone or any other communication mode, must be first reported to Central Communications. These folks will be responsible for following the procedures listed herein and for making ALL the appropriate notifications

The following procedures are intended to be a guide for personnel who must contact responders and in-house staff for accident/incident notification. Accidents/Incidents herein classified as “Serious” and as “Code 10”. Each classification requires specified contacts that must be initiated and documented.

Includes all contracted flexible services taxi trips and Northway Express.

SERIOUS ACCIDENTS/INCIDENTS

q  Any injury requiring medical transport.

q  Disabling damage to any vehicle that must be towed from the scene.

NOTE: If either of the above criteria is met, a post-accident drug and alcohol test may be required!

FOR SERIOUS ACCIDENTS/INCIDENTS CONTACT:

1.  Police, Fire, and/or emergency medical service as appropriate

2.  Street Supervisor closest to the scene

3.  Manager of Safety or designee

4.  Transportation Superintendent or designee-Closest to the incident

5.  Transportation Superintendent of employee or designee

6.  Director of Transportation or designee –(Must contact CEO and Director of Marketing or designee)

7.  Director of Maintenance or designee

CODE 10 ACCIDENTS/INCIDENTS

q  A Fatality

Five or more people injured and treated at and/or transported from the scene.

Mechanical failure resulting in an accident.

Bus Fire (either requiring evacuation or where there were visible flames).

A Pedestrian Accident ( where the pedestrian was knocked down by contact with a CDTA vehicle)

FOR CODE 10 ACCIDENTS/INCIDENTS CONTACT:

1.  Police, Fire and/or emergency medical service as appropriate

2.  Street Supervisor closest to the scene

3.  Manager of Safety or designee Refer to “Note 1” herein

4.  Transportation Superintendent or designee-Closest to the Incident

5.  Transportation Superintendent of employee or designee

6.  Director of Transportation or designee ( Must contact CEO and Director of Marketing or designee)

7.  Director of Maintenance or designee

A CODE 10 SECURITY INCIDENT

Will be declared by the responding Supervisor or Radio Control if the incident involves:

q  A bomb threat

q  A hijacking/hostage

q  A weapon used

q  A suspicious substance/package

q  Any physical violence involving a CDTA employee

FOR CODE 10 SECURITY INCIDENTS CONTACT

1.  Police, fire, and/or emergency medical service as appropriate

2.  Street Supervisor closest to the scene

3.  Director of Risk Management or designee Refer to “Note 2” herein

4.  Transportation Superintendent or designee -Closest to the Incident

5.  Transportation Superintendent of employee or designee

6.  Director of Transportation or designee (Must Contact CEO and Director of Marketing or designee)

7.  Director of Maintenance or designee

IN ADDITION FOR ALL SERIOUS ACCIDENTS/INCIDENTS OR CODE 10 ACCIDENTS/INCIDENTS OR SECURITY INCIDENTS THE FOLLOWING

EVENTS MUST OCCUR:

1.  Radio Control must notify all supervisory personnel that a CODE 10 exists when

Declared-

2.  Radio Control must send the Radio Log as a SUMMARY REPORT to the E-Mail

Group-

“CDTA ACCIDENT/INCIDENT NOTIFICATION”

This E-mail need not be done immediately, it should be sent at some point after the emergency part of the event is handled and details are ascertained- To Facilitate this E-Mail – An E-Mail Group will be established- “ CDTA ACCIDENT/INCIDENT NOTIFICATION” – when the e-mail is sent to this group, it will go to all appropriate managers automatically.

3. Radio Control must provide in the Radio Log Report the

following key information :

q  The location of the accident/incident

q  The bus number involved

q  The Operator’s name and ID

q  A brief description and current status of the accident/incident

q  That a CODE 10 was declared

The Manager of Safety will contact the Director of Human Resources for CODE 10 Accidents/Incidents and is responsible for all contact with the New York State Public Transportation Safety Board, (Day) 485-2448 – (Night) 485-7002 and will contact the CDTA Claims Manager as appropriate.

NOTE 1: It is imperative that contact be made with the Manager of Safety for all Code 10 accidents/incidents. The Manager of Safety is responsible for the overall management and follow- up relative to these accidents. Interoffice memos will be distributed to each division designating a back-up person (Supervisor of Safety) during the absence of the Manager. In the event that the Manager of Safety, or designated back-up, cannot be reached immediately then contact any other Supervisor of Safety. An Accident/Incident meeting “Code 10” criteria must be reported to the NYSDOT Public Transportation Safety Board within 90 minutes.

The Director of Risk Management will contact the VP (Administration & Finance) for CODE 10 Security Incidents

NOTE 2: It is imperative that contact be made with the Director of Risk Management for these incidents. The Director will be responsible for the overall management and follow up relative to these incidents. In all cases when the Director cannot be contacted contact the Manager of Safety, who will serve as the back-up person. In the event that the Director or Safety Manager cannot be reached immediately then contact any Supervisor of Safety and Training.
