(Checks payable to Wings Conservatory, Inc.).
• The $500.00 deposit check will be HELD only and not deposited.
• There will be new billing/payment plans this upcoming year details to follow.
• Deposit is non-refundable if your child does not participate in the 2012-2013 season.
Costumes/Workshops/YAGP: Please note there will be additional workshops and performances. Costume fees, competitions and workshops are not included in your tuition. Please remember to budget this season for these additional expenses.
Summer Audition DVD: If your child needs to create a DVD for her/his summer auditions, please call the studio to schedule a private lesson. Depending on the requirements for the DVD, this may take from 1 to 1 ½ hours to complete. You are welcome to film the DVD yourself. If you are unable to film the DVD, there is separate charge for filming and DVD editing which can be discussed.
Pointe Shoe Consultation: There is a $75 consultation fee if you wish for Andrea Kramer, artistic director, to accompany you at your pointe shoe fitting.

1 Ballet Technique class per week (60 minutes)

2 Technique classes, 1 Stretch (flexibility training) class per week

Division II: $6,100 per year
3 Technique classes, 1 Pointe class, 1 Modern class and 1 Pilates/Yoga class per week (stretch may be needed at an additional cost)

¨ If your child (Division II or III) is enrolled in NJDTE modern classes, he/she may choose to forgo Ballet Forte’s modern class. This will result in $500 reduction in tuition. Please check box if your child participates in NJDTE modern classes and is choosing not to attend Ballet Forte’s modern class.

Rules & Dress Code
Studio Rules
1. Arrive promptly to class (late student’s will be asked to observe class).
2. No talking during class, please raise your hand for questions.
3. No food or drinks in class, except water bottles.
4. No leaning on the barre during class.
5. Ask permission to use the restroom (unless it’s an emergency).
6. Hold finishing positions until the instructor indicates you may stop.
7. Give instructor full attention.
8. Teasing and gossip will NOT be tolerated.
Dress Code
1. Ballet class dress code is mandatory (see chart below).
2. Wear hair up neatly in a bun to all ballet classes.
3. No street or warm up clothes in class.
4. No jewelry, except small stud earrings.
5. New pointe shoes must be checked by Artistic Director, Andrea Kramer, before being worn to class.
6. Division III may wear ballet skirts to Monday classes (sheer, a solid color, mid-thigh length or shorter).
7. Modern class attire: form fitting dance clothing, knee pads are mandatory this year, socks, hair off of face (bun, pony tail, braid, or pig tails).

Notes to Parents/Guardians
• Pick-up/drop-off of students is preferred to minimize congestion in studio (except Pre-Forte students).
• Students picked up more than 20-minutes late are not the responsibility of Ballet Forte at Wings Conservatory, Inc.
• For discussions with the director or with an instructor, please call or email the office to schedule an appointment.
• Injured dancers are encouraged to sit and observe classes.
• Please email in absences.
• Please provide updated and complete medical and emergency information as needed.
Releases
Ballet Forte at Wings Conservatory, Inc. will be referred to as BF in the following releases.
Liability Release:
I hereby certify that my child is in normal health and capable of participating safely in BF’s programs. I will notify BF if the participant has any health problems. I am aware that dance training and the associated athletic exercises therein may place unusual stress on the body and carry with it the risk of physical injury. On behalf of my child and myself (and if I am no longer a minor, on my own behalf), I assume all risks and hazards incidental to the conduct of the program. I understand that if my child is injured, they are required to come watch their classes and take notes and I am still responsible for their full tuition. Picking up my child more than 20-minutes after class or rehearsal is my responsibility and not the responsibility of BF. BF is a full year program which requires a full year commitment. Tuition quoted is for a full year attendance. If my child decides to leave the program before June, I am liable for the full year tuition.
Publicity Release:
I hereby authorized BF to record the student’s picture and voice on photographs, films, and tapes, to edit these recordings at its discretion, and to incorporate these recordings into movie and sound films on tapes, radio or television broadcast programs. I also give my permission for BF to use and license others to use these materials in any manner or media whatsoever. BF is permitted to use these materials for publicity, advertising and sales promotion and to use the student’s name, likeness and voice and biographic or other information in connection with them. I acknowledge that no promises of compensation are made by BF for such use.
