INSTRUCTIONS

  1. Pleaserefer to the Public Sector Panels of Consultants (PSPC) Terms of Listing before completing this Renewal Application Form.
  1. You will need the following information to check the Renewal Application Form:
  2. Firm’s particulars
  3. Firm’s professional personnel qualification details
  4. Track record and client’s assessment details
  5. Details of staff nominated for Code of Buildable Design course

This Form may take you 30* minutes to fill in (*The average time required per panel application).

  1. Please gothrough the Renewal Application Form and fill up the declaration portion:-
  2. If you do not have any amendments/additions to the Form, please just submit the same Form back to BCA.
  3. If there are any amendments/additions to the Form, please make the necessary changes in ink on the Form and attach the supporting documents (e.g. Firm’s ISO, PII, Practicing Certificate). If there are changes to the track record, please fill up Part 3 of Application Form, with supporting documents and submit all the information to BCA.
  4. If there is a change in panel, please fill up the Application Form instead and submit the Application Form and relevant documents to BCA.
  1. Please note that the PSPC Terms of Listing (including criteria for each panel), Application Form, Frequency Asked Questions, GIRO form can be downloaded from click Contractors Registry, followed by Public Sector Panel of Consultants.
  1. A non-refundable processing fee is payable for each panel renewed. Payment can be made byNETs, Cashcard, Credit cards (Master/Visa) or GIRO (existing account holder only).
  1. You may wish to submit the Form and relevant documents via post (for GIRO accounts holder only) and/or in person to BCA.

I authorise the Authority to conduct any enquiries on the particulars furnished herein as the Authority deems fit, and accept that neither the Authority nor its officer shall be held liable for any loss, injury or damages howsoever caused by the Authority's processing of and decision on this application.
I understand that the provision of a false declaration or false information is a serious offence and may also result in penalties to my firm including the suspension of my firm from participating in public sector tenders.
