Position Summary: Working within an integrated team approach, the Community Mental Health Worker (CMHW)-Shared Care – Adult will contribute to the overall improved health status of patients within a Primary Health Care team through the provision of short term and brief treatment using mental health clinical counselling interventions and approaches. Services are provided to the adult population within a framework of mental health clinical practice, including consultation, assessment, and therapeutic interventions. The CMHW – Shared Care - Adult will work collaboratively and interactively with the primary health care provider, functioning as a clinician, service coordinator, and liaison with other sectors within the health care system as required. The CMHW – Shared Care - Adult will be required to function as an autonomous mental health practitioner within the Primary Health Care Team setting. The position of CMHW - Shared Care – Adult functions in a manner consistent with, and supports the Mission, Vision and Values of the Interlake-Eastern Regional Health Authority.
Qualifications:
B.Sc.M.H., RPN, RN, BSW, B.H.Ec (Family Studies) or B.M.R.
An equivalent combination of education and experience may be considered.
Licensed or eligible for licensure with professional designation if and as appropriate to educational background and a member in good standing.
Comprehensive knowledge and range of intervention modalities, including brief solution focused therapy, Cognitive Behavioural Therapy, and Motivational Interviewing.
Knowledge of mental health issues, including signs, symptoms, treatment and prognosis of mental illness and emotional distress
Knowledge of primary care principles.
Knowledge of co-occurring disorders, i.e. addictions and mental health, developmental delay and mental health, and the related impact on family dynamics
Knowledge of community resources and procedures related to accessing the available services
Knowledge of Psychiatric Disorders/Clinical Syndromes
Knowledge of Pharmacology
Knowledge of Recovery Model/Empowerment Theory
Knowledge of Mental Health Act
Minimum of three (3) years mental health clinical experience.
Satisfactory employment record.
Excellent interpersonal and communication skills.
ASIST Certificate (Applied Suicide Intervention Skills Training) required.
Demonstrated ability to problem-solve complex situations and manage sensitive issues.
Crisis intervention and bereavement skills.
Collaborative guiding principles in order to work within a Primary Care setting as a member of an interdisciplinary team.
Demonstrated skills working with diverse populations and presenting issues.
Advocate for and educate others on the Shared Care philosophy and program mandate.
Skilled at working with people with co-occurring mental health disorders.
Proficiency in Microsoft Office applications (Word, Excel, and PowerPoint).
Ability to travel to meetings at various locations as needed.
Flexible scheduling is required to balance client needs and group work.
Completes and maintains a satisfactory pre-employment security check.
