RA-4 (PHA) Housing Choice Voucher Residents

“60-Day Notice of Apartment CONTRACT Rent

Increase”

Form RA-4 (PHA) is used to notify the resident(s) and the Public Housing Authority provider that the CONTRACT (apartment) rent will be increasing within 60-days of the expiration date of the initial term of the Rental Agreement. This will amend the “RENT” section of the current Rental Agreement.

After the initial one-year term of the Rental Agreement for Housing Voucher residents, the Rental Agreement automatically converts to a month-to-month Lease. It is not required that we “renew” for an additional year. Please refer to “Rental Agreement Instructions” for additional Lease information.

Complete as follows:

1.  Fill in the date, unit #, name(s) and address of the Resident(s) apartment. Be sure to include the city, state and zip code where indicated on the form. Then enter the apartment community name where indicated.

2.  Complete the total NEW rental amounts and the effective date as applicable to the term of the new or updated Rental Agreement.

3.  Be sure that the Property Manager signs the form.

4.  Send the original form to the Resident(s). Send a Photocopy to the relevant Housing Authority. Also be sure to make a photocopy of the form and place it in Resident's file on top of the current Rental Agreement. This proves the Resident(s) were properly notified.

5.  Thirty (30) days prior to the Rental Agreement expiration date and whether or not the Resident(s) contacts the Manager’s Office or responds to this Notice, form RA-1amt “Rental Agreement Renewal Addendum” must also be completed and sent as the final notification of any “contract” rental change. Please note: If a NEW full Rental Agreement will be completed, form RA-1amt will not be required. For example, if an adult were being added or deleted from the household this would cause a new Rental Agreement to be completed.
