HEAD COACH:

SEASON ENDING REPORT

Each head coach of each sport is required to submit a written report to the Athletic Director within 3 weeks after the conclusion of a season. All requested information is to be submitted on the forms provided. This report will be made available to all administrators and possibly school board members. The report is to include yet not limited to:

PART 1

  1. Equipment needs for next season
  2. Recommendations for modification of facilities
  3. Long range plans for the sport (if any)
  4. Schedule recommendations for the future
  5. Referee recommendations

PART 2

  1. Season results (team or individual)
  2. Statistics ie. Team record, individual achievements, tourney results etc.
  3. Performance records
  4. School award winners
  5. Letter winners
  6. Participations awards
  7. Managers
  8. Honorary awards

PART 3

Evaluation of assistant coaches is mandatory and a signature from the head coach and each assistant coach is required before completing the report.

PART 4

Inventory of all equipment and supplies in detail is needed. Attach a detailed list to this report noting the quantity of each item. Attach an additional list of the equipment and supplies needed for next season.
