Leadville/Lake County Fire Department values your health and wellness and demonstrates this by providing evaluations in the Heart Disease Prevention Program (HDPP). The Human Performance Clinical/ Research Laboratory Staff areeager to continue to assist you in Discovering Your Healthy Lifestyle. Please understand that in order to be tested in this program you must be asymptomatic for cardiovascular disease. (If you have had a heart attack or stroke, please contact Tiffany at 491-7035 or .)

In addition, please understand that participation in the HDPP is not intended as a replacement for your regular medical care; therefore, if health risks are identified, you will be referred back to your primary care provider for follow-up.

Please read carefully!

  1. Email me at to get your lab requisition form. I will email the lab requisition form to you.
  1. Prior to your appointment time at CSU, arrive at the PVH/UCH for your blood draw and urine collection. You do not need an appointment to have your blood at urine collected at the UCH/PVH lab. It is walk-in/first come, first serve. We recommend having your blood and urine collection approximately 2 hours prior to your CSU appointment.
  1. You need to take this form with you for blood and urine collection. Avoid delays by having this form in hand and complete when you arrive at the UCH/PVH lab. I apologize that we were unable to post this form.
  1. Please fast for 12-14 hours prior to your blood collection. Please DO NOT eat anything until after you have had your blood drawn. Drink lots of water.
  1. Please take your medications as prescribed.
  1. Locations and hours of operation for the UCH/PVH labs may be found at the following websites: and The easiest site for this is the Express Lab located off of Prospect Road.
  1. Please check the RBC cholinesterase (RBCC) box. If you check any additional boxes on the lab requisition form, you will personally be responsible for the payment of additional tests (outside the established agreement with Leadville/Lake County Fire Department).
  1. The PVH/UCH will bill us directly. If you receive a bill for the blood and urine collection, this is an error. Contact me immediately (email: ) and let me know! Do not pay the bill.

Your evaluation will consist of one visit to our location in Fort Collins and a follow-up appointment. More detailed information about these appointments is on the pages that follow.

What to bring:

  1. Clothes (t-shirt and shorts) and shoes for exercising
  2. Swim suit

Refrain from any strenuous exercise 24 hours prior to the visit.

The data collected from you, including the health history form, will be kept confidential and may be accumulated for future heart disease prevention research. We would like to ask that we might use your data for this purpose. We would also like to ask about re-contacting you about future research. We will talk with you about this when you arrive and if you decide you would like your data to be used for research, we will ask you to sign a consent form during your visit.

Testing day schedule:

Approximately 2 hours before your CSU appointment, arrive at the PVH/UCH lab to have your blood and urine collected. After you have had your blood and urine collected, pleases eat a light, low fat meal.

Upon arrival at CSU, please park in the designated parking area (see map on website). We will provide you with a parking pass.

Upon your arrival at the lab you will be greeted by a HPCRL staff member and given an overview of the procedures for the visit. Please come prepared with clothes and shoes appropriate for exercising (preliminary and secondary portions of the visit) and a swimsuit (for the preliminary portion of the visit). For ease of placing EKG electrodes, female participants should wear a halter top/sport support without wires. Private locker room facilities with showers and towels are available for your use during all of the evaluation periods.

The preliminary portion of the visit will evaluate your pulmonary function level, strength, and flexibility. We will also determine your body fat percentage by the underwater weighing technique and skinfold procedures. This portion of the visit will take approximately one and one-half hours.

The secondary portion of the visitwill be the cardiovascular assessment. A thorough cardiorespiratory examination and review of your resting ECG tracing will be performed by a physician prior to exercise testing. This test will evaluate your overall health and fitness level. Prior to taking this test, we will ask for your written consent specifically for this test. This portion of the visit will require approximately one hour.

Preliminary and secondary portion may be switched to accommodate our physician’s schedule.

The final follow-up visit will be an individual consulting session to discuss your test results in detail. Your assigned staff member will provide feedback on your exam results and offer ideas and recommendations for implementing any needed changes to help you achieve your personal health goals. This visit will last approximately one hour and will be done at Leadville/Lake County Fire Department or at the HPCRL.

Leadville/Lake County Fire Department receives a form signed by the physician indicating that your ability to perform firefighting duties with either pass or fail.
