GUIDANCE ON USING THE CHILD PROTECTION RECORD FORM (CPRF) [1]

  1. Keep the information on the CPRF factual and focused and only include information that has been recorded in the client casenotes.
  1. Ensure the information you put on the form is clear and understandable to others, e.g. do not use a CPO’s name such as Mr Jones unless you make it clear what role Mr Jones has e.g. CPO, form teacher, etc.
  1. If you submit the front sheet as a written record e.g. to follow up a verbal report of concern, to a CPO or external person with CP responsibilities, ONLY do this once it has been checked by the Centre Manager/Scheme Manager/ Service Manager, Supervisor (this can be done over the phone) or PHL. In these situations the ‘Reported to’and ‘Signed by’section should be completed
  1. If you and your Centre Manager / Supervisor decide that a ‘disclosure report’ of a serious or urgent nature should be made it is recommended that you also consult with the Practice Helpline. This is particularly important if the report is being made without client consent, as it may ultimately involve complex legal issues which the Centre needs to be aware of.
  1. If a client discloses several concerning things at the same time or over a period of time; yet through consultation with your Supervisor and / or Manager it is agreed only one part of the disclosure will be reported –you will need to transfer the information that will be disclosed to a new CPRF.
  1. Under the ‘Consultation Log’ section it says: ‘Record below details of any consultations regarding the case’. This should include everyone you have consulted e.g. Supervisor, Centre Manager, PHLand the times and dates of the consultation.
  1. Disclosure Reporting’ – this refers to the reporting of a disclosure made by the client to the counsellor. If you, the counsellor, report a client’s ‘disclosure’ (something they have shared with you) to others –the term ‘disclosure reporting’ is used.
  1. Under the ‘Disclosure Reporting’section it says‘To which agency should a disclosure report be made?’ – agency in this instance is any person, service or organisation outside of Relate e.g. School, Children’s Centre, Connexions office, Social Care etc. plus the name and role of the person to whom the report will be made e.g. John James CPO at Stanton Lane School.
  1. ‘Who will make the disclosure report?’ – this is for final clarification
  1. ‘Who else will be informed?’ – if it is agreed that another agency will be informed, state clearly who that will be.
  1. ‘Reasons why no disclosure will be made’ – for future referenceit is important to summaries the rationale if it is decided not to make a discloser report
