To All Employees: Important Notice Regarding Open Enrollment

On December 15, 2016, the Trustees of the NY44 Health Trust approved cost saving initiatives. The NY44 Health Trust, in an effort to contain costs of emergency room visits, will offer a 24/7 OnLine Care to three of the four geographical areas: Western New York, East Central New York, and Mid-Central New York. This will be introduced at each school and available to enrollees on July 1, 2017.

In addition to these savings opportunities, the NY44 Health Trust plan will implement a mandatory Mail Order program for maintenance drugs beginning July 1, 2017. The program will provide 90 days of medication at 2.5 times the co-pay, if applicable. Pharmacy Benefit Dimensions will administer the program, and work with enrollees to ensure prescriptions are filled with no interruption in service. Any enrollee who is currently taking one or more of these maintenance (longer than two months) drugs, will receive important information directly from Pharmacy Benefit Dimensions. The information will be mailed to enrollees' home addresses on May 22 and May 23. Please watch your mailbox! Any questions should be directed to Pharmacy Benefit Dimensions after you review the information provided.

New prescriptions written after July 1, 2017:

Pharmacy Benefit Dimensions does not do ongoing communication after the first initial fill. This is handled at the pharmacy level. After the first fill, the subsequent fill will reject , informing the pharmacy and member that they are required to fill by mail. There is a one-time override if the member "forgets" this is now a mandatory mail fill. Usually it is only up to a 30 day supply, allowing the member time to contact the Physician and have the script sent to Mail Order.

Please refer to Hamburg’s District Web Page for necessary Pharmacy Benefit forms for your maintenance drugs.

Any questions, please have the member contact PBD customer's service number (716) 635.7880 or 1.888.878.9172, Monday through Friday 8 a.m. to 8 p.m.

Thank you,
