My Rewards Program

A Healthier You™ would like to reward you for participating in the health improvement program! Available on the portal homepage, the Rewards box will help you track your incentive participation.

Step 1: Complete your health screening

  • Knowing your health numbers will give you information and suggestions on how to improve or maintain your health status.
  • Screening dates to be announced.

Reward for completion: 25 points

Step 2: Complete your health risk assessment

  • The health risk assessment takes about 15 minutes to complete and will give you information and suggestions on how to improve or maintain your health status.
  • You may participate in the health risk assessment beginning January 1, 2013 through October 31, 2013 to be eligible to earn incentive points.

Reward for completion: 25 points

**Please note that completion of the Health Risk Assessment AND a screening are required in order to redeem incentive points.

Step 3: Participate in wellness activities

  • Participate in a variety of activities to earn additional points. See below for a chart listing available activities and point options.
  • NOTE: Not all programs may be offered at your location.

Notes:

The My Rewards program is available to:

  • All Missouri Western State University employees with Blue KC medical insurance.
  • Spouses of Missouri Western State University employees with Blue KC medical insurance.
  • All medically covered employees and spouses may earn up to 75 points during a program year.
  • Incentive items are generally delivered within 2-3 weeks of placing an order.
  • All incentives may be considered a form of income by the IRS. Reporting this income is your responsibility.
  • Some employers include the value of gift cards in wages and deduct applicable taxes from employee pay checks. Your employer can advise you on their process
  • A minimum redemption of $25 is required.
  • Rewards not redeemed by November 30, 2013 will be forfeited.
  • Reward options include debit cards and merchant gift cards through the online redemption website available to you once you have completed your incentive steps.
