When you become an active dues paid member of NCMA, you are eligible for 24 hours’ coverage of our $10,000 Accidental Death and Dismemberment Insurance Policy. We hope that none of our brothers and sisters will ever have to use the insurance policy, but during our routine activities and work related duties, the chances are high. When you become a member of NCMA, we need for you to complete the AD&D Insurance Beneficiary Form attached and send it to NCMA along with your membership application. We will retain a copy in our office and submit the original to the insurance company.

This is a Blanket Accident Policy and for additional explanation and clarification of terms, responsibility and details of each benefit, payment of claims, any exclusions, please download, print and read the policy in detail. This policy gives you 24-hour coverage for accidental death and dismemberment with additional benefits for immediate family counseling benefits, coma benefits, emergency medical evacuation benefits, repatriation benefit and a safety device benefit. This is an accident policy covering a sudden, unexpected event that results in a covered loss listed in the policy by a covered NCMA member.

SUMMARY OF POLICY COVERAGE:

$ 10,000.00 in benefits for 24 Hours coverage of an accident that is a sudden, unexpected event that results in Injury or death to the Covered Person.

ACCIDENTAL DEATH AND DISMEMBERMENT BENEFIT

If Injury to the Covered Person results in any of the Covered Losses shown below, within the Time Period for Loss as shown in Schedule of Benefits, the Company will pay the percentage of the Principal Sum shown below for that loss. The Principal Sum is shown in the Schedule of Benefits. If multiple losses occur, only one Benefit, the largest, will be paid for all Covered Losses due to the same Covered Accident.

ADDITIONAL ACCIDENT BENEFITS

Any benefits payable under these Additional Accident Benefits shown below are paid in addition to any Accidental Death and Dismemberment benefits payable, unless otherwise excluded or indicated under the terms, conditions, and exclusions of this Policy.

Bereavement and Trauma Counseling Benefit

Benefit Amount $150 per session

Maximum Number of Sessions 10 sessions

Maximum Benefit Per Covered Accident $1,500

Coma Benefit 1% per month of the Covered Person’s Principal Sum

Emergency Medical Evacuation Benefit

Maximum Benefit Per Covered Accident Up to 100% of the Usual and Customary Charges

Repatriation Benefit

Maximum Benefit Up to 100% of the Usual and Customary Charges

Safety Device Benefit Seat Belt: $5,000

Air Bag: $2,500

Please complete the Beneficiary Form attached and send it to our Membership/Digital Media Officer Carl Richard,address noted above, so we can keep your account noted and updated. If there is a need to change the beneficiary at any time, just go to our website, download, print and complete a new formand send it to our Membership/Digital Media Officer Carl Richard. We will forward it on to the insurance company.
