Instructions to Applicants

ELIGIBILITY RULES:

Any high school student (grades 9 through 12), providing his or her amateur standing is maintained, is eligible to make an application for this scholarship. Students may receive multiple awards of this scholarship to a maximum of $2000.00. Students must meet the following requirements:

1.Be 18 years of age or under, as of the August 1st of the current season;

2.Maintain a legal residence in the State of Florida;

3.Be a member of a bowling league certified in the Suncoast USBC, Inc. Association within one of the following centers Countryside Lanes, Dunedin Lanes, Hudson Bowl, Lane Glo Bowl, Lane Glo North and Strike City;

4.Be a member in good standing for the current season.

APPLICATION REQUIREMENTS:

1.Fill out page 1 entirely and sign it;

2.Have your Bowling Coach fill out page 2 entirely and sign it;

3.The applicant is to write an essay with a minimum of 500 words, but not to exceed 1000 words, saying why you wish to attend college or technical/trade school and what your future plans are;

4.Submit a copy of current grades or transcript;

5.Provide two personal reference letters of recommendations;

6.Submit application postmarked no later than- May 1st, 20__.
