Guidelines for the self-evaluation:

Please provide a self-evaluation statement on your job performance during the review period. The self-evaluation should address the extent to which you have fulfilled the duties and obligations set forth in the following areas:

You should also include self-identified goals for the coming year and reflect on your progress in achieving the mutually agreed upon goals from the prior year.

Your thorough and timely participation in the evaluation process will help facilitate a fair and comprehensive review of your accomplishments since the last evaluation. The completed form should be submitted to your supervisor at least two weeks before your scheduled review meeting.

D.  Planning & Review (Integrates planning, outcomes assessment, and review as a part of fulfilling responsibilities (e.g., SLOs, SSOs & unit outcomes). Coordinates responsibilities with others to advance District’s Educational Master Plan, Board of Trustee’s and Superintendent/President’s goals. Uses the College’s Educational Master Plan as a framework to communicate context within and across areas. Encourages active, timely involvement of appropriate faculty, staff and others in decision-making processes.)
