8.5.1 Guide to Recording and Reporting Work Related Injuries/Illness
Injuries/Illnesses that need to be recorded in the First Aid Treatment Record Book
Any injury or illness that occurs out of and in the course of work which prevents or inhibits a worker from carrying out their normal duties or which obviously requires first aid treatment, ( For example; cuts that require cleaning/dressing, swelling that requires ice, physical symptoms like dizziness, nausea, anxiety, aches & pains etc.). First Aid Attendants, or persons rendering first aid, should always complete the first aid treatment record book at the time treatment is provided and based on their observations at that time. In some cases it is acceptable to complete the treatment record book where no treatment or examination is provided, i.e. when information is provided over the phone or when the patient did not wish to be examined. However, in this case the report should specify “information only, no exam conducted”. A copy of all completed First Aid Treatment Records must be forwarded to Human Resources immediately for database entry.
WCB Reportable Injuries/Illness
A reportable injury/illness is one that occurs at work, and in which any one of the following conditions is present or subsequently occurs:
1. the worker loses consciousness following the injury/illness;
2. the worker is transported, directed or recommended by the first aid attendant or other representative of the employer to a hospital or clinic;
3. the injury/illness is one that obviously requires medical treatment;
4. the worker states his/her intention to seek medical attention;
5. the worker has received medical treatment for the injury/illness;
6. the worker is unable by reason of the injury/illness to return to his/her usual job function on any working day subsequent to the day of injury/illness;
7. the injury or accident resulted in the breakage of a prosthesis, eyeglasses,
dentures or hearing aid;
8. the worker or WCB has requested that an Employer’s Report be sent to WCB.
Where none of the conditions listed in 1 to 8 above are present, an injury/illness is minor and not required to be reported to the WCB unless one of those conditions later occurs.
First Aid Report - Form 7A
Where the injury/illness is reportable as per 1 to 8 above and the worker has reported to the First Aid Attendant for assessment/treatment, then the First Aid Attendant will:
· complete Form 7A including an accurate description of patient assessment from primary and secondary surveys, treatment provided and the “Worker’s Statement of Injury”, (use back of form if required);
· submit Form 7A to the supervisor for submission to HR and WCB;
· refer worker to Supervisor for completion of accident investigation and Forms 6A & 7

Workers’ Report of Injury - Form 6A
For any reportable injury as noted above, the workers’ supervisor will ensure that the worker completes a Form 6A report , (providing he/she is fit to do so), as soon as possible after the injury. This report should never be completed by someone else for signature by the injured worker. The completed form 6A will then be forwarded to HR.
Employer’s Report of Injury or Occupational Disease - Form 7
For any reportable injury as noted above, the workers’ supervisor or manager will ensure that a Form 7 is completed with as much information as possible, as gathered from interviews with the employee and any witnesses. The supervisor must sign the Form 7, forward it to HR for follow-up and submission to WCB. The WCB Regulation specifys that this be done within three days from the time the employer is informed of the workers’ injury. If there is any doubt as to whether or not the injury/illness occurred as stated, this must be indicated on question #2a with an attached note if necessary, otherwise, the supervisor/manager should contact HR for assistance.
Accident Investigation Report
For any reportable injury as noted above, the City of Nanaimo Accident Investigation Report must be completed. The Supervisors/Mgr. should initiate the investigation as soon as possible, with the assistance of an OH&S Committee member, (the OH&S Coordinator will also assist with investigations as required). An Accident Investigation Guide is available to assist with the investigation and form completion. The goal is to try and determine the basic underlying causes as well as the immediate causes of the accident. Then to implement or recommend corrective action(s) to try and prevent future similar accidents. The completed report should be forwarded to HR within 5 days from when the supervisor/manager is informed of the accident.
