REGISTERED GENERAL NURSE
Job title:Practice Nurse
Responsible to:Practice Manager and Dr Gail Wetmore, the nurse team lead
partner
Hours:20 hours per week
Salary:Dependent on experience
Main purpose of the job
To provide high quality nursing services for patients of the practice and to be a part of a team committed to supporting each other, practice and community team members in the delivery of patient care.
The responsibilities of the role are outlined below and whilst comprehensive other roles and functions may be identified by the partners or practice manager and may be included in the job description if it is felt to be appropriate.
Education and training
The practice is committed to personal continued professional development for all team members. Training will be provided as appropriate.
Professional responsibilities
Maintain professional registration and ensure that confirmation and up to date status is held at the practice.
New patient screening – this includes
Taking and recording, blood pressures, weight, height and body mass index
Urinalysis recording
Lifestyle and health promotion information
Peak flow measurement and recording
Relevant history and social factors
Vaccinations and injections – administration and recording
Routine injections to include Vitamin B12, Depo Provera
Childhood vaccinations as per the childhood vaccination programme
Travel vaccinations to include comprehensive travel advice
Influenza vaccinations as part of the annual vaccination programme
Dressing and wound management – to include
Removal of sutures/clips
Dressing wounds and leg ulcers
Liaison and consultation with nurse colleagues, local wound care nurse specialists, doctors where appropriate re dressings, taking swabs, need for antibiotics
Cervical cytology screening and well woman care – to include
Cervical smears
Lifestyle advice
Breast self examination
Contraception and menopause advice
Involvement in and with the practice nurse team to maintain and improve take up rates of the practice population
Long term condition management – to include
Monitoring long term conditions such as asthma, diabetes, COPD, CHD as appropriate and using the clinical system templates
Ear syringing – in line with the Patient Group Directions
Phlebotomy – using vacutainers and syringes
Smoking cessation advice
Domiciliary visits – to undertake domiciliary visits where a need has been identified for example, influenza vaccinations, ECGs, spirometry
Patient Health Promotion and Education –
To be involved with the practice nurse team in promoting health information in the surgeries, treatment rooms, nurse consult rooms and St Peters Surgery
Infection control – to include
Sterilisation
Liaising and working with other team members to ensure the highest standards of infection control are established and maintained within the practice
Stock control and management – to include when required
Involvement in stock control, ordering, rotation and management as and when required.
Awareness of the need for cost effective purchasing when involved in this area
Confidentiality and Safe Practice -
Maintenance of the highest standards of confidentiality in all matters relating to the patients, doctors, staff and the Practice
Maintain an interest in professional changes and developments in nursing and health related issues
Recording all information onto the practice clinical system and ensure accurate and comprehensive data
Identify personal needs in relation to future professional development and professional profile
To take part in nurse team meetings to develop and consolidate the work of the practice nurse team
Any other tasks that are identified and requested by the partners or practice manager
