POSITION SUMMARY: Provides technical assistance in the documentation and clarification of medication history lists. Conducts patient and/or caregiver interviews to obtain the most current medication history. Processes, enters, and interprets physician/nurse medication histories and completes full history documentation in computerized system. Clarifies information through communication with the providers, patients, and other pharmacies. Works with multiple medication automation systems. Assists, consults, contributes, executes, and participates in the daily activities of the Pharmacy Services Department to insure that optimum pharmaceutical care is provided to all patients. Assists staff pharmacists and director of pharmacy in all functions to maintain smooth operation of the department.Maintains the RXM database inconjunction with IT department.

QUALIFICATIONS:

EDUCATIONAL REQUIREMENTS

The Medication History Technician must be a high school graduate, be at least 18 years old and possess a good command of the English language. The Medication History Technician must have strong math capabilities, strong communication skills, and ability to work independently under the supervision of Registered Pharmacists.

The Medication History Technician must be a Registered Pharmacy Technician with the State Board of Pharmacy or be eligible to become registered. An applicant may be a Licensed Practical Nurse or Registered Nurse registered with the State.

National Pharmacy Technician Certification (CPhT) is preferred. If applicant is waiting for scheduling of test, test must be successfully completed and certification received within one year of assuming this position.

Three years of experience as pharmacy technician is required.

ENVIRONMENTAL FACTORS ABOUT THE JOB

1.Be able to move throughout the department and the hospital.

2.Possess proficient manual dexterity to use various office machines, such as, but not limited to, a telephone, typewriter, computer, calculator and Pyxis.

3.Access medications and other related supplies from various locations throughout the hospital.

4.Lift medications, supplies, IV fluids, and medication bins weighing approximately 25 pounds to waist or shoulder level.

  1. Prepare various drug preparations using a horizontal flow hood.
  2. Adhere to clean room dress code.
  3. Be able to push/pull several types of wire carts throughout the hospital. Load weight of one cart exceeds 75 pounds.
  4. Must possess the ability to multi-task and accept change.
  5. Interact cooperatively and act as part of the healthcare team with all employees, patients, caregivers, retail pharmacy personnel, nursing home personnel, physician, clinical staff, and visitors.
  6. Demonstrate and use aseptic technique to prepare sterile medications.
  7. Be able to read.
  8. Must possess proficient oral/written communication skills in the English language to communicate with patients, visitors, staff and physicians on a frequent basis.
  9. Must possess excellent visual acuity to read physician orders.
  10. Must possess excellent people interactive skills and customer service.
  11. Must possess strong mathematical abilities to calculate appropriate doses when preparing medications for patients.
  12. Must possess emotional stability to work in an extremely stressful environment that requires high levels of concentration to meet minute to minute deadlines.
  13. Strong time management, organizational skills, problem solving skills, and attention to detail. Ability to work effectively and efficiently under tight deadlines, with high volumes, and multiple interruptions.

room as the 7 a.m. IV person 10 times per year. Score based on supervisory
documentation of significant errors in the above processes. Significant errors
include wrong dose, wrong drug, wrong fluid and wrong patient.
5. Employee is familiar with normal Pyxis procedures. Restocks accurately and
in a timely manner. Must fill Pyxis 25 times per year. Score based on
number of errors in the Pyxis procedures.
6. Employee can perform all routine procedures using Meditech inventory
system including performing issues, running critical item reports and stock
out reports, creating orders (manual and automatic), receiving orders and
placing stock on shelves. Uses issue forms accurately and consistently.
Score based on number of times Employee works as 10:30, and 9:00 aux.
7. Must be familiar with all Pyxis applications to include, Connect, ASys, med
stations, PARx and how to maintain the equipment and trouble shoot.
8. Replenishes medications in emergency drug containers and floor stock
areasaccurately
9. Answers the telephone, identifying self and department. Directs calls to
appropriate personnel.
10. Answers requests at the window and makes stat deliveries to patient care
and ancillary areas when requested.
11. Keeps pharmacy areas clean, neat, and well-organized. Keeps pharmacy
equipment clean.
12. Completes and documents all assigned medication storage area
inspections at least monthly. Identifies and replaces outdated and unusable
medications.
13. Obtains allergy information with reactions from patient or caregivers. Enters
Information accurately into the computer system. Consults with pharmacist
If there is a question regarding the allergy information provided.
14. Processes patient medication history and enters information accurately into
Into computer system. Obtains and enters last dose information. Prints
report and reviews with patient for accuracy.
15. Contacts pharmacist to clarify questions regarding histories.
16. Works with nursing, physicians, and clinical staff as part of the patient care
team.
17. Completes all documentation as required for monitoring the medication
history program
18. Changes are made to RXM library accurately and as directed by the
pharmacist
19. Works with IT as needed if revisions on medication reconciliation
documented are needed. Informs director and department members of
changes in documents.
20.Works with nursing, pharmacist, and physician to resolve discrepancies with
home medications.
