This is a four-part application. All four parts must be completed and submitted together by the May 4, 2015 deadline. The applications must be mailed or hand-delivered to the following address:

Scholarship Notification Procedures:
- All applications must be received, with all four required parts, by Monday, May 4, 2015. It is the student’s responsibility to ensure the application package is complete and received on time. Incomplete or late applications will not be considered.
- Scholarship recipients will be notified by June 1, 2015. The scholarship recipient will be recognized during the June 4 Henry County School Boardmeeting to be held at the Henry County Administrative Building in the Benny Summerlin room at 6:00 p.m. Scholarships will be forwarded directly to the college prior to the start of the 2015-2016 fall semester. This is a one-time scholarship in the amount of $1,000. The scholarship will expire on October 30, 2015 and will not be re-issued.
