The Appendix K is a form that must be filled out and received by Oregon State Police (OSP) a minimum of two weeks prior to installation of the livescan unit.
The “Main Point of Contact” is the individual that the Oregon State Police, CJIS Division would contact for problems, setting up meetings, and who is in charge of the livescan when it is up and running. Not necessarily the individual who is arranging the unit to be installed.
The “Secondary Contact” is an individual that the Oregon State Police, CJIS Division can get in contact with if the Main Point of Contact is unreachable.
“IT Point of Contact” is the IT on site or who the agency depends on for the livescan(s) and network technical issues.
“Secondary IT” is in case the first IT cannot be reached.
“Jail Commander/Manager” is important contact information if the Oregon State Police, Identification Service Section is unable to reach anyone one of the contacts above and it is an urgent matter.
“Livescan Phone Number” is the phone number where one of our techs can reach someone at all times if we receive a card that has problems or concerns. This number is crucial and should be a number that is manned at all times during your working hours. Some agencies, like the jails, have booking rooms and are 24hr agencies. The number provided must be the booking room or a number that is manned 24hrs. Agencies that are not 24hr please make sure the phone number(s) are answered readily by someone who will be familiar with the livescan.
All of the above contacts will be sent urgent emails from time to time as deemed necessary because of network shut downs for maintenance, new equipment installs, AFIS issues, etc. which will interfere with transmission to OSP. It is extremely important the agency receives urgent emails regarding these matters as they arise. This way OSP knows the agency has received the email and at least one of the contacts will act on it. These emails are usually requested to be posted by the livescan units so those taking fingerprints are aware the system is down and not to transmit.
If this is a new install your Livescan will be installed with the current tables. However, if you are connecting your livescan to a Jail or Records Management system they must have the current tables as well which the Livescan Coordinator can send you upon request.
Some of the questions on the Appendix K can be answered by your IT person e.g. who your internet provider is, and bandwidth.
