Math & Science Scholarship Application

A Scholarship Fund of the Allegan County Community Foundation

The Perrigo Company funds two $500 scholarships annually in support of Allegan County Area Technical & Education Center students who intend to pursue an education in Math or Science. The scholarship is to be used to defray college costs (tuition, books, room/board) in the student’s freshman year. Scholars will be selected based on academic achievement, extracurricular activities/community service, technical achievement and staff recommendation. This scholarship is not renewable.

Eligibility

§  Applicants must be a graduating senior currently enrolled in an Allegan County Technical & Education program with a minimum 3.0 Tech Center GPA.

§  Applicant must plan to attend post-secondary college or training at a Michigan private or public community/junior college, a four-year degree granting institution, or a vocational training institution that grants a degree, certification or license.

§  Applicant must major in an applied math or science related program.

o  TYPED: The application must be typed.

TRANSCRIPTS: Include 1 (one) copy of your high school transcripts (obtained directly from your school counselor with signature and seal). Include your college transcripts if you have taken college courses.

o  RECOMMENDATION: Complete the top portion of the staff recommendation form (page 5) and give it to your Tech Center teacher by February 28, 2014.

o  PAGE IDENTIFIER: Enter the last 4 digits of your social security number on each page of the application. Your name should appear only on the application cover sheet and the Staff Recommendation form. (The Review Committee will not see any applicant names on the documents they review.)

o  SIGNATURE: Sign the application prior to submitting.

COPIES: Submit 4 (four) copies, of your completed Tech Center application (beginning with page 2) (one original and 3 copies). The Tech Center Office will provide access to their copy machine if you contact them in a timely manner to set up an appointment for making copies. Do not staple the applications; paper clips or clamps may be used. No binders or folders should be submitted.
